Description

This is a combined synopsis/solicitation for commercial items prepared in accordance with the format in FAR 12.6 streamlined procedures for commercial items, in conjunction with FAR subpart 13 and as supplemented with additional information included in this notice. This is a Firm fixed Priced Contract, Small Business Set-Aside requirement. Only qualified offerors may submit quotes. Solicitation VA249-12-Q-1226 is issued as a Request for Quotation (RFQ) and constitutes the only solicitation, a written solicitation will not be issued. NAICS Code is 238220 and the size standard $14M. Any firm that does not meet the capability and size standard under this NAICS code should not submit a response to this notice. In order to be considered for a government award, the firm must be registered in SAMS@www.sams.gov and be capable of providing labor, service, manuals, tools, material and equipment to successfully complete monthly and annual preventative maintenance on the Dialysis Clinic Water Treatment System listed in the attached statement of work. This service is required for the VA Tennessee Valley Healthcare System (TVHS), 1310 24th Avenue South, Nashville, Tennessee, 37212, in accordance with the terms, conditions, special provisions and schedules herein. In order to be considered responsive, the firm must offer a quote on all itemized items and total base year including all option years pricing as provided below. Base Year: Award date - 09/30/2013 Option Year I: 10/1/2013 - 09/30/2014 Option Year II: 10/1/2014 - 09/30/2015 Option Year III: 10/1/2015 - 09/30/2016 Option Year IV: 10/1/2016 - 09/30/2017 Equipment Make and Model: MAR-COR PURIFICATION RO) MODEL: RO,23G-7.5K,208V,60HZ,75%. SERIAL # 1271851 PLUS ALL SUPPORTING EQUIPMENT (TANKS,PUMPS,PIPING,ELECTRONIT CONTROLS, ECT) Award will be made to the lowest price quote which conforms to the requirements within this solicitation.
