Description
This is a combined synopsis/solicitation for commercial items prepared in accordance with the format in Subpart 12.6, as supplemented with additional information included in this notice. This announcement constitutes the only solicitation; proposals are being requested and a written solicitation will not be issued. The solicitation number is W91YTV-12-T-0232. This acquisition is issued as a Request for Quote (RFQ). The solicitation document and incorporated provisions and clauses are those in effect through Federal Acquisition Circular 2005-58. Federal Acquisition Regulation (FAR) Clause 52.211-6, Brand Name or Equal applies to this acquisition. Vendors shall comply with FAR 52.211-6 if submitting other than the Brand Name product. This requirement is Unrestricted and the North American Industry Classification System Code (NAICS) for this procurement is: 325120; and the small business size standard is 500. Monchrief Army Community Hospital, Fort Jackson, South Carolina 29207-5720, has a need for the refill and delivery of Government owned lung diffusion gas mix with various medical gases as listed below. SUBSTITUTED ITEMS: If you plan to provide other than the item cited in the purchase description, provide in your response the brand name and catalog number you intend to furnish and the characteristics of the substituted item that are equivalent to the brand name. Also, provide all documents which clearly describe the item (s) so that it/they can be evaluated by the end user. Failure to submit the substituted item information before the closing date listed below will eliminate the vendor from all further consideration. Items otherwise equal may not satisfy the requirement due to differences in material, composition, size, environmental application, etc. If an item is furnished which does not meet the requirement the item will be returned at the vendor's expense for a full refund. Final decision as to award will be based on lowest price technically acceptable (technical capability will be determined by the end user). Award of this solicitation is pursuant to FAR 52.232-18, Availability of Funds (Apr 1984), Funds are not presently available for this contract. The Government's obligation under this contract is contingent upon the availability of appropriated funds from which payment for contract purposes can be made. No legal liability on the part of the Government for any payment may arise until funds are made available to the Contracting Officer for this contract and until the Contractor receives notice of such availability, to be confirmed in writing by the Contracting Officer. Medical Gases Statement of Work 1. Description of Services: This requirement is for the procurement of medical cylinder gas cylinders to be utilized in a Military Medical Treatment Facility (MTF), Moncrief Army Community Hospital (MACH) Jackson,SC. Procurement of medical gases is vital to the daily operation functions of MACH in order to provide a high standard of medical healthcare to the military community. The Contractor shall provide service consisting of products of listed government owned medical gas cylinders to include hydrostatic testing as needed to the locations of MACH; BLDG 500 Stuart St on Ft. Jackson, South Carolina. Additional inclusions (i.e. includes field modifications such as safety corrections by the Contractor or requested by the Government). 2. Hours of Performance: a.Delivery Hours are every Thursday between 11:00 a.m. and 11:30 a.m. b. Weekly deliveries Monday through Friday excluding federal holidays. A scheduled day agreeable between the Government and Contractor will be established, preferably on Wednesday. 3. Response Time Standards: Delivery will be made weekly. In the event there is an emergency delivery needed, the requirement must be fulfilled within 24 hrs. upon receipt with of the order. This could be at any time not previously stated. 4. Technical Data Availability: Contractor must deliver medical gas cylinders in compliance with State and Federal Codes. All cylinders filled with gas product must be in compliance with State and Federal Codes. All cylinders must have lot numbers labeled on each cylinder delivered. Delivery ticket must be signed by the Customer after products are delivered. Purity of medical gas oxygen must be no less than 99% when delivered. 5. Contractor responsibility: Include any special personnel qualifications required on contract employees,(i.e.: speak English, professional licenses, drivers licenses, security clearance, permits, uniform or badge requirements, neatness or cleanliness requirements, etc.). When written certifications or licenses are included here, cross reference the requirement to a Technical Exhibit, and require the contractor submit the documentation as a required report. Contractor must inform COR/Alternated COR of any changes in delivery schedules one week in advance. 6. Administrative Control: The assigned COR/Alternated COR is the only authorized individual to make changes to the delivery schedules after contract award. The COR/Alternate COR will contact the assigned Contracting Office to inform of changes in delivery schedule requesting a contract modification if applicable. Contractor shall report any delivery delays to the assigned COR/Alternate COR 24 hours in advance if applicable. Contractor will submit a combo in WAWF for payment in a timely manner. 7. Legal holidays: The date observance by the Federal Government of the following holidays: New Years Day (1January), MLK Birthday (3rd Monday in January), Washington's Birthday (3rd Monday in February),Memorial Day (last Monday in May),Independence Day (4th of July),labor Day (1st Monday in September), Columbus Day (2"d Monday in October),Veteran's Day (11th of November),Thanksgiving Day (4th Thursday in November), and Christmas Day (25th of December). 8. Security Control: Contractor must enter Ft. Jackson and presenting the following: a valid driver's license of all individuals entering the military installation. The government anticipates awarding a single award for this requirement. Services are to be delivered at: Moncrief Army Community Hospital, BLDG 4500 Stuart St., Fort Jackson, SC, 29207-5720. All offerors must be registered in the Systems for Award Management System and or prior to award, and lack of registration shall make an offer ineligible for award.
