Description
THIS IS A COMBINED SYNOPSIS/SOLICITATION FOR COMMERCIAL ITEMS PREPARED IN ACCORDANCE WITH THE FORMAT IN FEDERAL ACQUISITION REGULATION (FAR) SUBPART 12.6 "SOLICITATION FOR COMMERCIAL ITEMS", AS SUPPLEMENTED WITH ADDITIONAL INFORMATION INCLUDED IN THIS NOTICE. THIS ANNOUNCEMENT CONSTITUTES THE ONLY SOLICITATION. QUOTATIONS ARE BEING REQUESTED AND A WRITTEN SOLICITATION WILL NOT BE ISSUED. This solicitation number is RFQ-RT-12-00154 and the solicitation is being issued as a 100% small business set-aside competition Request for Quotation (RFQ) using FAR Part 12 Commercial Items and FAR Part 13 Simplified Acquisition Procedures. The solicitation document and incorporated provisions and clauses are those in effect through Federal Acquisition Circular 05-55, effective February 2, 2012. The associated North American Industry Classification System (NAICS) Code 624190 - Other Individual and Family Services, which has a size standard of $7.0 million, is applicable. A firm fix ed-price contract is anticipated to result from award of this solicitation. This is a Performance-Based procurement. The procurement is for Employee Assistance Services. See the SF-18 (Attachment # 1) for a list of contract line items. A. BACKGROUND - Federally employed workers at the Western Ecology Divison (WED) and their dependents are subjected to the stressors similar to those of other workers of the nation and, at times, may need an outlet and counseling for those stressors which affect the workplace and production of research and support staff. Among the stresses of government workers which could affect the workplace and workplace production are issues of family and domestic partners, finance, safety, substance abuse, workplace violence and interpersonal conflict, medical issues, grief, depression, career options, community opinion, social website issues, identity theft, critical incident stress, and more. It is the intent of this contract to provide for relief of stressors through information and emotional relief in order to maintain a safe and healthy workforce and place of employment. Medical treatment, prescriptions and procedures are not included under this contract. The Division employs ap proximately 85 full-time Federal workers in three branches located in Corvallis Oregon and Newport Oregon. See Attachment # 2, Statement of Work for a description of requirements for the services to be acquired and for the dates and places of acceptance. B. PERIOD OF PERFORMANCE - Base Period: October 15, 2012 through October 14, 2013, Option Period 1: October 15, 2013 through October 14, 2014, Option Period 2: October 15, 2014 through October 14, 2015, Option Period 3: October 15, 2015 through October 14, 2016, Option Period 4: October 15, 2016 through October 14, 2017. (See Statement of Work for duty hours). EPA WED non-duty hours are: a. Daily, Monday through Thursday, 5:00 p.m. to 8:00 a.m. the next day. b. Weekends, 5:00 p.m. Friday to 7:00 a.m. Monday. c. Federal Holidays as described: (1) Columbus Day, (2) Veterans Day, (3) Thanksgiving Day, (4) Christmas Day, (5) New Year's Day, (6) Martin Luther King Day, (7) Presidents' Day, (8) Memorial Day, (9) Independence Day, (10) Labor Day). C. TERMS AND CONDITIONS - The Statement of Work (SOW), Evaluation Criteria, Pricing Sheet and any other information pertaining to this requirement can only be found at the EPA website listed at the end of this synopsis/solicitation. In accordance with FAR 52.204-7(b)(1) Central Contractor Registration, all prospective awardees shall be registered in the CCR database prior to award, during performance, and through final payment of any contract or purchasing agreement resulting from this solicitation. All offerors are to include with their offers a completed copy of provision 52.212-3, Offeror Representations and Certifications ?Commercial Items. In addition to FAR 52.212-3, clauses 52.212-1, 52.212-2, 52.212-4 and 52.212-5 apply to this acquisition. See Attachment # 3 for a listing of these provisions and clauses and all other FAR provisions and clauses applicable to this acquisition. Any applicable wage determination will be made a part of the resultant contract. As of August 5, 2012, Wage Determination No. 2005-2439, Revision No. 13 (Attachment # 4) is the applicable wage determination and will be made a part of the resultant contract (see Attachment 3). If the wage determination is revised prior to award, the wage determination will be updated. For this solicitation, all evaluation factors (see FAR clause 52.212-2 of Attachment # 3) other than cost or price, when combined are significantly more important than cost or price. The EPA will make award to the responsible offeror whose offer conforms to this solicitation and is most advantageous to the Government, cost or price and other factors considered.
