Description
This contract action was accomplished using other than competitive procedures because the procurement is awarded to a sole source under the Test Program for Certain Commercial Items (FAR 13.5). See posted J&A for further information. MEMORANDUM FOR RECORD FOR OTHER THEN FULL AND OPEN COMPETITION PRIORITY 03 1. Contracting Agency: Southern Regional Contracting Office, San Antonio Military Medical Center (SAMMC), Fort Sam Houston, TX 78234 2. Description of Action: Priority 03 urgent order, for Tonier Affinity total Shoulder and Reverse Shoulder and Reverse Fracture Implants for a patient with severe rheumatoid arthritis, dislocation, and fracture of her left shoulder in the Preoperative Nursing Services. This action has an unusual and compelling urgency that could be detrimental to the medical health care mission or could result in loss of life. 3. Description of Products/Services: This requirement is for supplied needed for surgery at San Antonio Military Medical Center (SAMMC), Ft Sam Houston, TX. Implants for a Cardio surgery. The implants are needed to reconstruct the left shoulder joints. 4. Authority Cited: 10 U.S.C. 2304(c)(2) is implemented by FAR 6.302-2(a)(2) Unusual and compelling urgency. The agency's need is of such an unusual and compelling urgent nature that without the service, the medical healthcare mission will be compromised unless the agency is permitted to limit the number of sources from which it solicits quotes. -Pursuant to FAR Part 5.202(a)(s), Exceptions, the unusual and compelling requirement will not be advertised. -Pursuant to FAR Part 13.106(c)(1), Soliciting competition, price and availability will be obtained telephonically. 5. Reason for Cited Authority: The above statutory authority is cited because this is a true, urgent request that has been verified by COL Joseph M. Paulino Chief, Perioperative Nursing Servicesand Janet Rindorf on 19 Sept. The Tonier Affinity total Shouldrer and Reverse Shoulder and Reverse Fracture Implants by 20 Sep 2012. Not receiving this Implant would compromise patient care as the physician has determined the surgery urgent. 6.
