RFP-245-M12-0009 Addendum 001 INFORMATION SECTION 1. Updated RFP DUE DATE: September 26, 2012, Wednesday 2. Updated RFP MAILING/PHYSICAL ADDRESS: Department of Health & Human Services Chinle Comprehensive Health Care Facility Division of Acquisition Management & Contracts Attn: Tanya M. Johnson, Contracting Officer Off Highway 191 & Hospital Drive PO Drawer "PH" Chinle, AZ 86503 3. Kayenta Health Center's Emergency Room annual volume is around 17,000 patients. QUESTION/ANSWER SECTION 1. QUESTION: What is the current coverage schedule now? How many physicians are scheduled in 24 hours? ANSWER: Current coverage schedule and the number of physicians is proprietary information and cannot be released, because this RFP is asking offerors to provide their best solution/approach to providing 24/7 ER coverage. 2. QUESTION: Are there Kayenta physician employees covering the ED currently? If so, will these employees be scheduled in for this new contract? ANSWER: There is on Lead ER Physician, but the RFP is requesting for the Contractor to provide 24/7 coverage. This Physician will not be scheduled, that is the Contractor's responsibility to provide 24/7 coverage. 3. QUESTION: How many ambulance "ride alongs" occur during the course of the year? ANSWER: This is quite variable with some months having none, and some months when it turns out to be weekly. For planning purposes, a range of 2-4 per month would be a reasonable average. Since these are almost entirely obstetrics patients, there is no predictable pattern. 4. QUESTION: How many times was an ED physician "called in" to utilize during contingency coverage during the course of the year? ANSWER: Call-backs for reasons other than ambulance ride-alongs have been extremely rare (probably less than 2 per year). Total call-backs, including for ambulance ride-alongs, average less than one per month, due to the availability of the double-coverage physician, and the fact that peak volume coincides with the presence of the double-coverage physician. 5. QUESTION: How many mid-level's or RN's are available to cover the ED? During which shifts are they presently utilized the most? ANSWER: Currently, all providers are physicians, however, there is no prohibition against mid-level ED providers. RN staffing includes 3 nurses at 7 AM every day; M-F a 4th nurse starts at 8 AM and a 5th nurse at 11 AM. Saturday and Sunday a 4th nurse starts at 11 AM. At 11 PM every night, staffing returns to 3 nurses until the morning. 6. QUESTION: Can you please provide a breakdown of volume during the course of the day? ANSWER: We don't currently have statistics by the hour, but the pattern follows the nursing coverage described above. So, 11 PM to 8 AM the volume is clearly the lowest; weekdays the busiest hours are late morning to early evening. Monday and Fridays tend to be the busiest, and volume is definitely higher during the winter months. 7. QUESTION: Is the CPAR form something you fill out or do you need it completed by Contracting Officers on contracts that we currently hold & sent in by them to you? Is this like a Past Performance Questionnaire that you need us to send to our current contracts to complete on our behalf? ANSWER: This will be completed after services have been rendered by the Contracting Officer's Representative, then the Contracting Officer, then lastly by the Contractor. This is a performance rating system. 8. QUESTION: Is there an incumbent. If so, who are they and where do they live? ANSWER: This RFP is for a new contract in its entirety. The current contractor is Health Care Partners. 9. QUESTION: What ancillary staff is available? ANSWER: Refer to answers to question 2 & 5. 10. QUESTION: What is the most frequent patient diagnosis or reason for presenting to the ER? ANSWER: The Kayenta ED is the only acute care facility within a large area, so the presenting patient problems include the full range of possibilities and typically are the same as in any ED. We do not have a report of numbers sorted by patient problem, but most common include: chest pain, abdominal pain, respiratory illness, musculoskeletal pain, gastrointestinal illness, and headache. Because the availability of clinic appointments are limited, a higher number of low acuity patients seek care in our ED than is typical of other ED settings. Overall, about 2% of patients are Triage level 1, 10-12% level 2, 25% level 3, nearly 60% level 4, and 4% level 5. 11. QUESTION: We see from your website that there is housing provided for Kayenta permanent employees. Would we be able to rent some of these facilities for our contract staff? If so, what is the cost? ANSWER: There are no Government quarters available for this requirement. There are several hotels and trailer parks as optional places to reside.
