Description
Performance Work Statement (PWS) for Chapel Youth Coordinator Space and Missile Systems Center (SMC), 61st ABG/HC has a requirement for a Chapel Youth Coordinator. This is a combined synopsis/solicitation for commercial items prepared in accordance with the format in FAR Subpart 12.6, as supplemented with additional information included in this notice. This notice constitutes the only notice. A written solicitation will not be issued. Solicitation FA2816-12-T-0007 Chapel Youth Coordinator is a request for quotation (RFQ). This solicitation document incorporates provisions and clauses of those in effect through Federal Acquisition Circular 2005-61, Defense Circular Number 20120906, and Air Force Acquisition Circular 2012-0330. Award will be made to the contractor who is able to supply a product that meets the specifications set forth in this solicitation, can meet the delivery schedule of 30 days or less After Receipt of Order (ARO), and is the lowest price. This proposed contract is being considered as a full and open competition. The NAICS Code is 813110 and the Size Standard $7M. The government is interested in all businesses that are interested in performing this requirement. A Firm Fixed Priced award will be made in accordance with the following Schedule: 1. DESCRIPTION/QTY: The service consist of the purchase of a Chapel Youth Coordinator who provides a spiritually based outreach program that infuses spiritual resiliency which includes emotional, relational, and spiritual needs of Middle and High School age students at Los Angeles Air Force Base, California. This service is targeted to teenagers whose parents and guardians have committed their lives to service with our armed forces. This program will appeal to the faith tenets of all chapel Christian denominations, as categorized by the Air Force Chaplain Corps. The quantity of this requirement will consist in a base year following wih four 1-year option as follows: •a) Base Year contract of 07 Months: •- Periof of Performance (PoP): 30 September 2012 - 30 April 2013 •b) Option Exercise One (1) of 12 months •- Periof of Performance (PoP): 01 May 2013 - 31 May 2014 •c) Option Exercise Two (2) of 12 months •- Periof of Performance (PoP): 01 May 2014 - 31 May 2015 •d) Option Exercise Three (3) of 12 months •- Periof of Performance (PoP): 01 May 2015 - 31 May 2016 •e) Option Exercise Four (4) of 12 months •- Periof of Performance (PoP): 01 May 2016 - 31 May 2017 2. When requested, the ombudsman will maintain strict confidentiality as to the source of the concern. The existence of the ombudsman does not affect the authority of the program manager, contracting officer, or source selection official. Further, the ombudsman does not participate in the evaluation of proposals, the source selection process, or the adjudication of protests or formal contract disputes. The ombudsman may refer the party to another official who can resolve the concern. (b) Before consulting with an ombudsman, interested parties must first address their concerns, issues, disagreements, and/or recommendations to the contracting officer for resolution.
