Description
Drawing and photos of site The purpose of this job is to repair damaged concrete curbs, stairs, and sidewalk at the following location. The David C Wynecoop Memorial Clinic is located at the Spokane Indian Reservation, Wellpinit, Washington, approximately 45 miles northwest of Spokane, Washington. The concrete stairs, curbs and concrete sidewalk at the base of stairs located at southwest corner of the clinic have minor cracks and broken curbs. The affected area is approximately 200 square feet with 13 flights of stairs as shown on attached site plan. Some photos depicting the damaged concrete are shown on Attachment #2 Physical address of job site: David C Wynecoop Memorial Clinic 6205 Agency Rd Wellpinit, WA 99040 Scope of Work: This applies to all damaged concrete within the circle indicated on the attached drawing. Photos are for the contractor's reference and may not cover the full extent of damaged concrete. 1. Clean the affected area with air compressor to remove all damaged and brittle concrete. Any damaged curbs within five feet of either side of the concrete stairs shall be removed. Thoroughly clean out the cracks by exposing the depth of concrete cracks. 2. Repair damaged concrete curbs ensuring the newly formed curbs are compatible with the existing curbs. 3. Apply epoxy crack sealant to seal all the cracks on sidewalk and stairs. 4. Apply two coats of Euclid overlay system or approved equal product on the stairs to provide non-skid surface. 5. Clean and restore the site. Performance Period: Complete all work within 30 days of notification to proceed. General Conditions: 1. The contractor shall ensure all works are performed in accordance with the applicable codes, safety standards, and adhere to the general provisions of the Contract documents. 2. Cutting, patching and repairing required by the work shall be the responsibility of the Contractor. 3. Should any part of the adjacent concrete or asphalt pavement area are disturbed, damages that results from work under the contract shall be promptly repaired at the expense of the Contractor. 4. The Contractor shall not obstruct access to the site and shall maintain the work site in good order, free of construction debris at end of each workday. The Contractor shall perform work at all times in a manner to ensure minimal obstruction to traffic and minimal inconvenience to the public. Notification and Inspection: 1. The Contractor shall keep the Project Officer notified and advised of all work to be done or work in progress sufficiently in advance so that proper inspection can be performed. The Contractor shall inform the Project Officer at least 5 business days prior to the requested date of final inspection to allow sufficient preparation and scheduling of Government personnel. 2. Contractor shall coordinate daily work schedule wi th the on-site Facility Manager, Steve Abrahamson, at 509 258-4517 X123. 3. The Contractor should examine areas and conditions under which work is to be performed and incorporate all factors or costs in preparation of bid to complete work. Failure of the Contractor to include all costs in preparation of bid will not be basis for additional costs or delays. Warranty: The Contractor shall provide a one-year warranty on all materials and workmanship or otherwise stated.
