- Standard Form 1449 with Brand Name Justification, & Pricing Schedule attached. This is a combined synopsis/solicitation for commercial services prepared in accordance with the format in Subpart 12.6, as supplemented with additional information included in this notice. This announcement constitutes the only solicitation; quotes are being requested and a written solicitation will not be issued. Solicitation number RFQ-KS-12-00033 is applicable and is issued as a request for quotation. The solicitation and incorporated provisions and clauses are those in effect through Federal Acquisition Circular 05-60. This acquisition is 100% set-aside for small business. The North American Industry Classification System (NAICS) code for this acquisition is 334519 (Other Measuring and Controlling Device Manufacturing) and the Small Business size standard for this NAICS code is 500 employees. This is a brand name only acquisition (Brand Name Justification, Attachment 1). The description of the commercial items is as follows: 1) M 192 Survey Meter, Part No.: 48-2945 - Qty: 1 2) Source-1 uCI Cs137 with stick-on label, Part No.: 4464-473-01 - Qty: 2 3) M 2221 Portable Scaler (with associated parts), Part No.: 48-2065 - Qty: 1 - Dial 202-159 50-500K CPM; 0-500 CPM - Rolled Handle - 39" "C" cable - Series "C" connector - D cell batteries (4 each) - Shoulder strap 4) M 44-20 collimator, Part No: 7032-051 - Qty: 1 5) CBL-C 39" only, Part No.: 40-1004 - Qty: 2 6) CBL-M221 to 9P RS232 8', Part No: 8303-434-8 - Qty: 2 7) Case-DSI-iM2300 BLK, Part No.: 2311063 - Qty: 1 8) M 44-20 3x3" IL DET, Part No: 47-1104 - Qty: 1 The Government intends to make award on an "all or none" basis. The selection resulting from this request for quotation will be made on the basis of the lowest priced quote from a responsible quoter with the ability to furnish all of the products and services specified in the Pricing Schedule (Attachment 2) within the required delivery schedule (September 26, 2012). It is the sole responsibility of the contractor to demonstrate its ability to provide all specified products and services. The Contracting Officer is not responsible for locating or obtaining any information not identified in an offeror's quote. Once the Contracting Officer has decided to whom an order will be issued, a unilateral order will be issued using a Standard Form 1449. A purchase order will be in existence once the Contracting Officer has signed the order. Wherever the words offer, proposal, offerors, or similar terms are used in this solicitation or in any applicable provision, they shall be read to mean quote, quotation, quoter, vendor or similar corresponding term to reflect that this solicitation is a Request for Quotation not a Request for Proposal or an Invitation for bid. Provision 52.212-1, Instructions to Offerors-Commercial Items, applies to this acquisition. The Contracting Officer will review quotes on the basis of information furnished by the quoter. The Contracting Officer is not responsible for locating or obtaining any information not identified in the quote. Telephonic inquiries will not be accepted. Quotes must be emailed, faxed, or mailed and are due on September 21 at 12:00 PM Central Daylight Time (CDT)and must include a copy of the quoter's completed business representations and certification, providing the business size, e-mail address, Dunn and Bradstreet number, CAGE code and Federal Tax ID number. In addition, quoters must be registered in the System for Award Management (SAM) database at www.sam.gov to be considered for selection. Quoters are required to submit completed copies of the provisions at FAR 52.212-3 and its ALT I, Offeror Representations and Certifications - Commercial Items, with their quotes. FAR clause 52.212-4, Contract Terms and Conditions Required to Implement Statutes or Executive Orders, Commercial Items, applies to this acquisition. INSTRUCTIONS Offers shall submit, by email, fax, or mail: (1) a complete quote using the PRICING SCHEDULE provided (Attachment 2). Failure to fully complete the provided PRICING SCHEDULE (Attachment 2) will cause an offeror's quote to be deemed nonresponsive.
