- Request No: D-2-D7-2A-AU-C00 This is a combined synopsis/solicitation prepared in accordance with the format in FAR 13.105 and as supplemented with additional information included in this notice. This announcement constitutes the only solicitation; quotes are being requested and written solicitation will NOT be issued. The closing date for price quotes is 12:00 pm (CST) on Monday, September 25, 2012. The associated North American Industrial Classification System (NAICS) code for this procurement is 334517. This requirement is a 100% Small Business set-aside and only qualified offerors may submit a quote. Preference will be given to SDVOSB and HubZone small businesses. The Internal Revenue Service requires the following item meets the specifications described below: X-RAY MACHINE - Brand may be equivalent to the following: •- Smiths Detection HI-SCAN 6046SI with 17" Color Monitor, HI-SPOT Automatic Dense Area Detection, HI-TRAX Image Enhancement, HI-Mat Organic/ Inorganic Stripping, 12-month standard warranty and includes installation, calibration, training, and radiation survey. •- 4-year extended warranty package to include parts, shipping, labor, travel costs, with an annual preventive maintenance visit including a radiation survey. •- Image store system, digital image management OTS Xtrain Operator Training Software •- 1 Meter entry/exit roller tables •- Shipping, Installation, One-Year Warranty, 1, EA; DELIVERY & INSTALLATION TO: Cedar Bend Professional Building 12309 N.
