Description
This is a combined synopsis/solicitation for commercial items prepared in accordance with the format prescribed in FAR Subpart 12.6, as supplemented with additional information included in this notice. This announcement constitutes the only solicitation; quotes are being requested and a written solicitation will not be issued. Solicitation FA4877-12-Q-0650 is issued as a request for quotation (RFQ). This solicitation document incorporates provisions and clauses of those in effect through Federal Acquisition Circular 2005-60, Defense Circular Number 20120906, and Air Force Acquisition Circular 2012-0330. This procurement is being issued as a total small business set aside. The Associated North American Industrial Classification Standard (NAICS) code for this request is 333512 with a size standard 500 employees or less. The results of this requirement will be a firm fixed priced (FFP) contract. The right to make multiple or no award is reserved in the event it is advantageous to the Government to do so. Items called for by this RFQ have been identified in the schedule by brand name or equal. The contractor shall provide the following Contract Line Item Numbers (CLINs): CLIN 0001- (1 Each) ShopSabre 3636 CNC Laser system to include the following accessories: a digitizing probe w/ single feedback wire to controller-For use with 3/8" Collet; a basic fixed dust skirt with 4" outlet for dust collection; a material height touch pad-measures material thickness for setting Z Axis height; a tool measuring switch for measuring new cutting tools; and a ShopSabre computer, Intel, Pentium Duo-Core Processor, 2GB memory, 320GB hard drive, 16x DVD-ROM drive, keyboard, mouse and monitor included (Minimum configuration). (End of Provision) 5352.223-9001 Health and Safety on Government Installations (JUN 1997) (a) In performing work under this contract on a Government installation, the contractor shall: (1) Comply with the specific health and safety requirements established by this contract; (2) Comply with the health and safety rules of the Government installation that concern related activities not directly addressed in this contract; (3) Take all reasonable steps and precautions to prevent accidents and preserve the health and safety of contractor and Government personnel performing or in any way coming in contact with the performance of this contract; and (4) Take such additional immediate precautions as the contracting officer may reasonably require for health and safety purposes. (b) The contracting officer may, by written order, direct Air Force Occupational Safety and Health (AFOSH) Standards and/or health/safety standards as may be required in the performance of this contract and any adjustments resulting from such direction will be in accordance with the Changes clause of this contract. (c) Any violation of these health and safety rules and requirements, unless promptly corrected as directed by the contracting officer, shall be grounds for termination of this contract in accordance with the Default clause of this contract. DM017 Davis Monthan AFB Marijuana Policy In accordance with 355 Fighter Wing guidance, all persons are prohibited from introducing, using, possessing, distributing, or purchasing marijuana on Davis-Monthan AFB for any reason, even if it has been medically prescribed. Contractors who violate this guidance shall be immediately detained by base security forces, who will then turn those persons over to the local authorities and those individuals found in violation shall be barred from the installation.
