- Linen Services Bid Sheet to include Usage data Specifications - Little Rock Job Corps Linen Services The Little Rock Job Corps Center is soliciting bids to furnish Job Corps linen services to all designated departments, for 2012-2013 contract year. If your company is interested, please submit a detailed proposal as per specifications. Bids must be returned with the following information clearly marked: Linen Service Bid #12-02 & Bidding Company's Name. All bids must be e-mail or mail to: lrjcpurchasing@hotmail.com or Little Rock Job Corps Center Purchasing Department Attn: Lavorce Crane Bid #12-01 6900 Scott Hamilton Drive Little Rock, AR 72209-3144 Bid proposals will be accepted until September 24, 2012 @ 5:00 p.m. In the event that a successful bidder to whom an items or items of this bid has been awarded should fail to supply the Little Rock Job Corps Center's requirement promptly and according to the specification and prices of the bid, the Job Corps Purchasing Department, reserves the right to cancel the award and purchase elsewhere. Little Rock Job Corps Center (d.b.a. Adams & Associates Inc.) 6900 Scott Hamilton Drive, Little Rock, AR 72209-3144 Tel. (501) 61-2518 TERMS AND CONDITIONS 1. Contract Period: The contract issued as a result of the bid request shall run October 1, 2012 through September 30, 2013. The linen price submitted shall continue in force during the contract period. 2. Services: It is the responsibility of the successful bidder to assure all linen products are freshly laundered and deliver on the designated day of each week. 3. Packaging: All linen shall be properly packaged to protect from being soiled prior to delivery. 4. Delivery: Deliveries shall be made on designated day as ordered; unless specified otherwise, and at designated hours specified by and arranged with the Purchasing Agent. It is the responsibility of the successful bidder to place all deliveries in the Warehouse except for rugs/scrapper which are delivered to the designated areas. All sales order /invoice slips must obtain the purchase order number, reviewed and signed by authorized receiving personnel for Job Corps. 5. Sanitation: All linen must meet all sanitary conditions, both as to premises and employees. 6. Substitutes: Linen Services shall be ordered by the Week (according to the Little Rock Job Corps' needs) by the Purchasing Agent or Finance & Administration Director. Items not on the quotation listing shall not be delivered without prior approval from the Purchasing Agent. If a substitute is necessary, delivery of an equal or lower price is permitted. 7. Samples: It will be the responsibility of the successful bidder to supply samples of all items upon request. 8. Payments / Credit: Payment of account will be made within 30 days of receipt of weekly invoices. All bills are to be sent to the Little Rock Job Corps Center, Attn: Finance Department, Purchase Order #, 6900 Scott Hamilton Drive, Little Rock, AR 72209-3144. The successful bidder will submit a consolidated, summary invoice on a monthly basis that includes billing for all received and accepted deliverables made during the period. 9. Insurance: The successful bidder shall secure and maintain in full force and effect insurance coverage issued by a responsible insurance carrier for: (i) general liability for an amount not less than $1,000,000.00 for any occurrence involving bodily injury or property damage; and worker compensation, in amounts required by federal and state laws.
