Description

This is a combined synopsis/solicitation for commercial items prepared in accordance with the format in Subpart 12.6, as supplemented with additional information included in this notice. This announcement constitutes the only solicitation; proposals are being requested and a written solicitation will not be issued. COMBINED SYNOPSIS SOLICITATION FOR VTC MAINTENANCE HDX 9000 SERIES A: This is a combined synopsis/solicitation for commercial items prepared in accordance with the format in Subpart 12.6, as supplemented with additional information included in this notice. This announcement constitutes the only solicitation; proposals are being requested and a written solicitation will not be issued. B. Solicitation Number: W564KV-12-T-0130 is issued as a: _X_ request for quotation (RFQ) ___ request for proposal (RFP). C. This solicitation document and incorporated provisions and clauses are those in effect through Federal Acquisition Circular 2005-43. D. This RFQ is not set-aside for any NAICS code or small business size standard. The Small Business Competitiveness Demonstration Program does not apply to this requirement. E. Schedule of Items/Services Required Polycom Premier Two Year Service (HDX 9000)4 Each Polycom Premier Two Year Service to Onsite Service Upgrade4 Each Service Implementation 1 Each Service Implementation 1 Each HDX 9000 Series Multipoint (MP) Plus Key1 Each All prices shall be submitted in (US Dollars or Euros) F. Description of requirements: See Specifications as listed in Attachment 01. G. Delivery and Acceptance: Date of Delivery: 45 Days after date of contract Delivery/Acceptance Location: 168 Romerstrasse Building 27 69126 HEIDELBERG Germany FOB Point: HEIDELBERG Germany H. The provision at 52.212-1, Instructions to Offerors -- Commercial, applies to this acquisition. I. The provision at 52.212-2, Evaluation -- Commercial Items does apply to this acquisition. The specific evaluation criteria are included in that provision, is at included in the Attachment. The award will be made to the lowest-priced technically acceptable quote received. J. Offerors are advised to include a completed copy of the provision at 52.212-3, Offeror Representations and Certifications -- Commercial Items, with their offer. K. The clause at 52.212-4, Contract Terms and Conditions -- Commercial Items, applies to this acquisition. L. The clause at 52.212-5, Contract Terms and Conditions Required To Implement Statutes Or Executive Orders -- Commercial Items, applies to this acquisition and is included in the attachment. M. All additional contract requirement(s) or terms and conditions that have been determined by the contracting officer to be necessary for this acquisition and consistent with customary commercial practices are listed in the Addendum to FAR 52.212-4 in the Attachment. N. There is no assigned rating in the Defense Priorities and Allocations System (DPAS) and assigned rating. O. The date, time and place offers are due: 1600 hrs (4:00 p.m.) 19 Sept 2012, CEST P. Submittal of Quotes: Quotes shall be submitted directly to the Point of Contact (POC): Donna Y. Ferguson, Contract Specialist. The quote shall be e-mailed to the POC at donna.y.ferguson.civ@mail.mil. Offerors are warned that failure to submit quote appropriately to the above listed e-mail address may disqualify the quote.
