Attached is the revised PWS as of 11 September 2012 Amendment 0002: CDC CCTV Maintenance This amendment to the combined synopsis originally posted on 22 August 2012 and Amendment 0001 posted 06 September 2012. See below for following changes. The Purpose of this amendment is to: 1. Change the solicitation quote due date and time from 13 September 2012 at 12:00 P.M. Eastern Standard Time to 13 September 2012 at 4:00 P.M. Eastern Standard Time 2. Change the contract line item number and price structure. 3. Make revisions to the Statement of Work. See attached PWS, dated 11 September 2012. 4. Change the evaluation criteria to include 52.217-5 Evaluation of Option Years 5. Change the NAICS code from 561612 to 561621. 6. Post vendor submitted questions and government response. 7. All other terms and conditions remain the same. Unit Price ___________; Quantity 2; Extended Amount _________________ CLIN 0002: Unscheduled Maintenance: Perform unscheduled maintenance on all Cameras, Monitors, and DVR's at all three CDC facilities. Price Per Labor Hour/Hourly Rate _________________________ CLIN 0003: Component cost not to exceed $10,000.00. The component cost shall not be a determining factor for evaluation. Inclusion of this CLIN is for informational purposes to establish a limit on costs incurred during the course of the year. Total (Extended Amount + Hourly Rate) _____________________ Basis for award is Lowest Price Technically Acceptable (LPTA). Partial quotes will NOT be accepted. Only one (1) award will be made as a result of this combined synopsis/solicitation. All offerors should timely notify the government of any changes in inventory and market availability that may preclude or delay performance of the delivery schedule and all any other terms and conditions stated herein. In addition, the government intends to make award (offer) to the responsive firm without discussions; prospective offerors are advised to submit their firm's best offer prior to the proposal (quote) closing date. Base plus option years will be evaluated according to Far Part 52.217-5. Customer Questions and Government Response: Can you provide statistics for the total number of service calls made either by facility or as a total? 36 While I understand that the cameras are interchangeable, are we going to be limited to direct replacement (brand and model specific) actions on the DVR's? As long as the replacement is comparable to the brand and model they are replacing. What size are the hard drives in the current DVR's? Information is currently unavailable. Currently all dvrs are operative. Does the base have a contractor's access badge or will the technicians haveto get passes on each visit? If the vendor send the same technician out we will can do a contractors base access pass. If it is a different technician each call we will get a pass each visit.
