Description
This is a combined synopsis/solicitation for commercial items prepared in accordance with the format in Subpart 12.6, as supplemented with additional information included in this notice. This announcement constitutes the only solicitation; proposals are being requested and a written solicitation will not be issued. Solicitation number W9114F-12-T-0054 is issued as a request for quotation (RFQ). Solicitation document and incorporated provisions and clauses are those in effect through Federal Acquisition Circular 2005-50, effective 16 March 2011. Not a set-aside. North American Industry Classification System (NAICS) code for this solicitation is 423450. The Europe Regional Contracting Office has a requirement to purchase one (1) Topcon TRK-1P Fully Automatic Combined Screening Station (Brand Name or Equal). If a quoter submits an Equal Product, quoter shall explain how the offered product meets the Brand Name product. Equal products of the brand name manufacturer must meet the following equipment salient characteristics: Factor I - Technical. TRK-1P Topcon Fully Automatic Combined Screening Station Europe 220V/50Hz - Topcon Medical Systems, Inc. (Brand Name or Equal) to include Installation and On-site Training at the US Army Health Clinic in Camp Darby, Livorno, Italy, shall, at a minimum: Combine 4-in-1 units to include Auto Alignment system: a) Auto Refractometer; b) Keratometer; c) Non-Contact Tonometer; and d) Optical Pachymeter. Handle pupils as small as 2 mm Include Installation and On-site Training at the US Army Health Clinic in Camp Darby, Livorno, Italy. Include 2ea operator and service manuals in English Include Manual alignment mode and Measurement mode in different colors, to include Rotary Prism Technology (Brand Name or Equal) for accuracy and reliability. At a minimum, include: Large Color Tilt Screen 6.5" VGA Thermal Printer Paper Cutter Connectivity and Compatibility to CV-5000 Automated Vision Tester. Minimum specifications: Measuring Range: for REF: Hyperopia: 0 to +22D (0.12D/0.25D steps) for Myopia: 0 to -25D (0.12D/0.25D steps) for Astigmatism: 0 to +/- 10D (0.12D/0.25D steps) Axial Angle: 0 to 180 degree ( 1 degree/5 degree steps) Target fixation: Auto fog System Measuring Range for Keratometer: Radius of corneal curvature: 5.00 to 10.00 mm )0.1 mm step) Corneal refractory power: 67.50 to 33.75D (0.12D/0.25D steps) Corneal astigmatic power: 0 to +/- 10D Corneal astigmatic axial angle: 0 - 180 degree Ocular Pressure Measurement: Range: 0 to 30mmHg/0 to 60 mmHg; Working distance: 11mm; External output terminal: RS-232x2, USBx1 Operating Temp: 10 - 40 degree celsius Main body travel: distance 40mm back/forth, 90mm sides, 30mm up down; chinrest travel distance: 60mm up/down electric. Factor II - Price. The Government will select the quote that is based upon the lowest evaluated price, technically acceptable quote. All CLINs shall have been priced by the vendor. If the vendor failed to price all CLINs, the quote will not be considered for award. ADDENDUM TO FAR 52.212-2 The Government will award a Purchase Order resulting from this solicitation to the responsible vendor whose quote conforming to the solicitation will be most advantageous to the Government, price and other factors considered. Award will be made on the basis of the low price technically acceptable quote. To be considered for award, quotes of Equal products including Equal products of the brand name manufacturer must meet the salient characteristics described above for each CLIN. EVALUATION STANDARDS. The quoter's technical quote will be evaluated using the following standards for a final rating: ACCEPTABLE: A quote that meets the Government's Salient Characteristics identified in the solicitation. UNACCEPTABLE: A quote that fails to meet the Government's Salient Characteristics identified in the solicitation. SPECIAL NOTE: All vendors must be registered in the Central Contractor Registration (CCR) prior to award, and lack of registration shall make an offer ineligible for award.
