- ATTACHMENT 3 WAGE DETERMINATION ATTACHMENT 2 CLAUSES ATTACHMENT 1 SOW RFQ STANDARD FORM 18 Synopsis: This is a combined synopsis/solicitation for commercial service prepared in accordance with the format in FAR Subpart 12.6, in conjunction with FAR 13.5, as applicable, and as supplemented with additional information included in this notice. This announcement constitutes the only solicitation; quotes are being requested and a written solicitation will not be issued. The solicitation number is HSCG34-12-Q-PIR016 and is issued as a Request for Quote (RFQ), unless otherwise indicated herein. The solicitation document and incorporated provisions and clauses are those in effect through Federal Acquisition Circular FAC 2005-52. The associated North American Industrial Classification System (NAICS) code for this procurement is 811310 with a small business size standard of $7.0 million. This requirement is a Small Business set-aside and only qualified offerors may submit a quote. The selected Offeror must comply with the following commercial item terms and conditions, which are incorporated herein by reference: FAR 52.212-1, Instructions to Offerors - Commercial Items, applies to this acquisition; FAR 52.212-3, Offeror Representations and Certifications - Commercial Items - the selected offeror must submit a completed copy of the listed representations and certifications; FAR 52.212-4, Contract Terms and Conditions - Commercial Items; FAR 52.212-5, Contract Terms and Conditions Required To Implement Statutes or Executive Orders-Commercial Items, paragraph (a) and the following clauses in paragraph (b): 52.222-21, 52.222-26, 52.222-35, 52.222-36, 52.222-37, 52.225-13, 52.232-34. All Contractors doing business with the Government must be registered with the System for Award Management (SAM). No award can be made to a company not registered in SAM. A Dun and Bradstreet number may be obtained free of charge by contacting Dun and Bradstreet at http://www.dnb.com/us/duns_update/ or 800.234.3867. Offerors are encouraged to go on-line at www.bpn.gov to complete the representations and certifications (ORCA). The U. S. Coast Guard, Honolulu, HI has a requirement for the services and/or supplies as listed below. Please email quotation and completed 52.212-3 Offeror Representations and Certifications-Commercial Items form to Base Honolulu (s), Attn: Gary Kansky at Gary.R.Kansky@uscg.mil as soon as possible, but no later than 11:59 p.m. HST, 12 September 2012. Phone for Gary Kansky: (808) 842-2826. Fax: (808) 842-2834. The contractor shall provide the following service as listed below from Attachment 1: 1. Contractor shall provide all required parts, labor, and equipment to repair USCG owned Grove RT58D mobile crane, serial number 83137 to operate within all manufacturer's and applicable safety standards. Cost shall include any transportation cost to move crane back and forth from contractor's repair facilities. 2. Crane Specifications: - Make: Grove; Model: RT58D; S/N: 83137 3. Work shall include but not be limited to the following: Inspect/grease all wear pads, pivot points, swing gear, swing bearing and chassis. Change engine oil and all filters. Inspect and repair hydraulic leaks in front and back, possible lines and or fittings. Boom chatters when retracting. Disassemble complete boom, repack extension cylinder, replace wear pads, replace retract/extend cables; adjust and shim boom. Replace 315' hoist line. Torque Swing Bearing/Turntable bolts to manufacturer's specifications. Calibrate PAT (DS 350 GW) System. R&R back up alarm. 4. Only manufacturer/factory authorized replacement part shall be used in repair. 5. All mechanical work shall be accomplished/supervised by a certified Grove activity. 6. Equipment Inspection: In order to provide an accurate quote, a full inspection (at no cost to the USCG) shall be performed on the crane at its current location at USCG, Base Honolulu, 400 Sand Island Parkway, Honolulu, HI 96819. 7. The Service Contract Act is applicable to this requirement. Please reference Attachment 3, Department of Labor wage determination. APPLICABLE CLAUSES: Attachment 2, clauses are applicable to any award resulting from this request for quotation. REQUIRED SUBMITTALS: Complete Standard Form 18, blocks 8, 11(e), 11(f), 13, 14, 15, and 16; and email your quote to Gary.R.Kansky@uscg.mil Also, please specify lead time to complete all work. QUOTE DEADLINE: Quote must be received by 11:59 P.M., 9/12/0212. RFQ EVALUATION: Responses to this RFQ will be evaluated based on the lowest-priced-technically acceptable quote. Technical acceptance will be based on vendor's ability to satisfy items #4 and #5 of scope of work. Documentation must be submitted to support compliance of technical requirements.
