Description
This is a combined synopsis/solicitation Request for Quote for commercial items prepared in accordance with the format in FAR Subpart 12.6, as supplemented with additional information included in this notice. This announcement constitutes the only solicitation; quotes are being requested and a written solicitation will not be issued. The solicitation number for this acquisition is NIAID RFQ 12-107 and the solicitation is being issued as Request for Quote. This solicitation document and its incorporated provisions are those in effect through the Federal Acquisition Circular (FAC 2005-60). This acquisition will be processed under FAR Part 13, Simplified Acquisition Procedures and is not a Small Business set aside, however, small, small disadvantage, minority, veteran owned, and HUB zone companies, etc. are encouraged to submit quotes. The North American Industry Classification System (NAICS) Code for this acquisition is 334510, which has a size standard in 500 of employees. The National Institutes of Health, National Institute of Allergy and Infectious Diseases (NIH/NIAID), intends to procure a portable wireless, handheld, blood analyzer and accessories. The portable wireless, handheld blood analyzer must provide real-time, lab-quality results within minutes, rather than hours. The portable wireless, handheld blood analyzer must be lightweight and features the following: a) single-use disposable cartridges, b) performs cardiac markers, blood gases, chemistries and electrolytes, lactate, coagulation, and hematology, c) transmit test results from bedside to desktop computer of patient records. d) certifiable for compatibility with existing wireless networks, e) fully integrated system with a comprehensive test menu. Item number/quantity/description: Line item 1: quantity 3, i-STAT 1 Analyzer Line item 2: quantity 3, i-STAT downloader recharger Kit Line item 3: quantity 3, i-STAT Power Pack Line item 4: quantity 3, i-STAT Printer Kit Line item 5: quantity 3, i-STAT Electronic simulator Line item 6: quantity 3, CDS Software Line item 7: quantity 25, cTnI, cartridges Line item 8: quantity 25, PT/INR, cartridges Line item 9: quantity 25, ACTc, cartridges Line item 10: quantity 25, CG8+, cartridges Line item 11: quantity 25, Creatinine, cartridges Line item 12: quantity 50, 6+, cartridges The following factors shall be used to evaluate offers : a) technical capability to meet required specifications, b) Price, c) delivery and d) warranty. Place of Delivery is NIH/NIAID, Rockville, Maryland 20852. The government will award a fixed price purchase order to the responsible contractor. The following Federal Acquisition Regulations provisions and clauses apply to the acquisition: FAR 52.212-1, Instructions to Offerors, Commercial Items; FAR 52.212-2, Evaluation - Commercial Items; FAR 52.212-3, Offer Representations and Certifications-Commercial Items; Far 52.212-4, Contract Terms and Conditions -Commercial Items; FAR 52.212-5, Contract Terms and Conditions Required to Implement Statutes or Executive Orders - Commercial Items; and FAR 52.232-33, Payment by Electronic Funds Transfer-Central Contractor Registration (Oct 2003). In order to be considered for an award, an offeror must have completed the online electronic Representations and Certifications located at http://www.sam.gov in accordance with FAR 4.1201(a). By submission of an offer, the offeror acknowledges the requirement that a prospective awardee shall be registered in the CCR at http://www.sam.gov prior to award, during performance, and through final payment of any contract, basic agreement, basic ordering agreement, or blanket purchasing agreement resulting from this solicitation [Note: Lack of registration in the Central Contractor Registration will make an offeror ineligible for award.] All responsible sources may submit an offer that will be considered by the Agency. Offers must be submitted no later than 4:00 P.M. Eastern Daylight Time on TBD.
