- Project Description The G.V. (Sonny) Montgomery VA Medical Center, VISN 16 NCO requires a Contractor to provide all labor, material, tools, equipment, transportation, and qualified supervision to perform Pipette Calibration Services at and for the Department of Veterans Affairs, G.V. (Sonny) Montgomery VA Medical Center, in Jackson, Mississippi all in accordance with the Scope of Work. General This procurement is a 100% Set-Aside for Small Businesses. Offers from other than certified Small Business firms shall not be accepted. STATEMENT OF WORK PIPETTE CALIBRATION SERVICE To assure that the Pipettes at the Department of Veterans Affairs, G.V. (Sonny) Montgomery VA Medical Center are operating properly and safely the Department of Veterans Affairs requires Service, Repair, and Calibration of Single Pipettes as presented to a Service Technician on site at the Department of Veterans Affairs, G.V. (Sonny) Montgomery VA Medical Center Lab area. The Contractor shall service or proceed with the following: 1.Follow Level 1 reporting on each single channel instrument. 2.All single-channel pipettes shall receive at least one seal/o-ring set per calibration. 3.All instruments shall be broken down and cleaned inside and out. 4.Pistons will be polished and/or lubricated as specified by the manufacturers. 5.All instruments shall be calibrated and adjusted. 6.All instruments shall be labeled with the date of calibration. 7.Shall provide individual pipette calibration data form for each pipette. 8.The data form shall be comparable with Level 1 reporting. 9.Service shall be billed after each calibration is completed. REPORTS: Upon completion of the calibration service and within ten (10) business days after each such service, a written report of inspection will be provided to the COR notifying the Department of Veterans Affairs of any discrepancies. Any unsafe conditions encountered during the calibration service will be reported immediately to the Contracting Officer's Representative (COR) or designee, advising the Department of Veterans Affairs as to whether the Pipettes units have been kept clean and lubricated where necessary. ANNUAL TEST: Safeties shall be subject to an annual running test. NOTE: The Contractor will provide Calibration Services ONLY and will not perform any maintenance. SCHEDULE: The Department of Veterans Affairs requires the Service shall be provided once a quarter or biannual according to the following: Quarterly Pipette Calibration and Service BLOOD BANK: 9 Pipettes: MLA adjustable volumes (1000ul, 500ul, 400ul, 200ul, 100ul, 50ul, 25ul, 10ul) 2 Pipettes: Tipmaster (50ul, 25ul, 12.5ul) Semi-Annually Pipette Calibration and Service MICRO: 2 Pipettes: #145 adjustable volume from Biomerieux (Vitek) volumes 145ul & 280ul 1 Pipette: Oxford 1 ml volume CHEMISTRY: 7 Pipettes: MLA adjustable volumes (100ul, 50ul, 25ul, 10ul, 1000ul, 500ul, 200ul) 2 Pipettes: EPP RES (1000ul, 500ul, 200ul, 100 ul, 50ul, 25ul, 10ul) ELECTROPHORESIS: 6 Pipettes: MLA adjustable volumes (500ul, 400ul, 200ul, 100ul, 50ul, 10ul) 1 Pipette: Microdispens 5ul 1 Pipette: Labnet 100ul HEMATOLOGY: 1 Pipette: Gilson (5000 ul, 2500ul, 2000ul) 9 Pipettes: MLA adjustable volumes (1000ul, 750ul, 500ul, 250ul, 50ul, 25ul, 10ul) GENERAL:The Contractor shall furnish all labor, equipment, supplies, parts, supervision, management and transportation necessary to perform Pipettes Calibration Service in accordance with (IAW) the specifications, terms and conditions of this contract for the Department of Veterans Affairs, G.V. (Sonny) Montgomery VA Medical Center for a period of one (1) year to commence October 1, 2012 through September 30, 2013, with four (4) option years for renewal. All work is to be performed in accordance with (IAW) Federal, Mississippi State and local laws and ordinances. IDENTIFICATION: Contractor's employees must have visible on their uniform, their name and company's logo. CERTIFICATION OF CONTRACTORS: Contractors shall be certified to perform Pipette Calibration. The Contractor shall furnish a copy of up-to-date and current State certification (showing categories) to the Department of Purchasing and Contracting and to the COTR upon request and prior to the start of the contract. CONFORMITY TO REGULATIONS: The Contractor shall conform to Federal, State and local regulations governing, Pipette Calibration Service. INTERFERENCE TO NORMAL FUNCTIONS: The Contractor may be required to interrupt their work at any time so as not to interfere with the normal functioning of the facilities, fire protection systems and pass of facility patients personnel, equipment and carts. HOURS OF OPERATION: The normal hours of operation for all facilities are 7:00 am to 5:30 pm, Monday through Friday. SCHEDULE OF VISITS: The Contractor shall provide a schedule of when Inspection Services shall be scheduled to the Contracting Officer Representative (COR) within fifteen (15) calendar days prior to the anticipated inspection date. Inspection tasks shall be scheduled so as to be performed on definite appointed days and in no event shall the Contractor perform inspections outside regular agreed upon work hours without prior approval from the COR. In the event climate or weather conditions make it unsuitable for inspection services, the entire service date will be re-scheduled to a date and time satisfactory to the Contractor and the COR. Postponement will not reflect any penalties assessed to the Contractor or to the Government. POINT OF CONTACT (POC): The Contracting Officer shall delegate the COR to monitor the Contractor's performance. REQUIRED CONTRACTOR REPORTING:The Contractor will sign in and out when performing Inspection services. The COR shall provide instructions as to the POCs and locations of sign-in/out. All Inspection Services shall be documented. The COR shall brief the Contractor on the preparation of this report. The Contractor is responsible for supplying, completing and submitting reports as required and requested by Federal, State or local ordinances which pertain to any duties contained in the contract. BASIS OF AWARD: The Department of Veterans Affairs intends on awarding a contract to a qualified firm that can meet the contract requirements and is the best overall value to the Government, price and other factors considered. CONTRACTOR SHALL SUBMIT ADDITIONAL INFORMATION AS REQUIRED IN SECTION 52.212-1.
