- AMENDMENT NO.: 0001 IS FOR SOLICITATION NO.: A12PS02366 IS AS FOLLOWS: 1. TO CORRECT THE STATEMENT OF WORK TO READ AS FOLLOWS: STATEMENT OF WORK Physical Therapy Services Kayenta Community School, North Highway 163, Kayenta, AZ is in need of a contractor to provide Physical Therapy Services beginning date of award to June 30. 2013. Ten students have Physical Therapy needs currently noted on their Individual Education Plan. Physical therapy is needed for two students at this time. Further, the contractor will be involved in a school-wide Child Find and Screening for an kindergarten students (total: 60 students). THE CONTRACTOR WILL: 1.Review student's medical records, physician referral, child study team referral, parent request and/or school psychologist evaluation recommendation for further testing; 2.Perform evaluation procedures to establish performance guidelines; 3.Provide formal written comprehensive test results of student assessment 30 days from date of evaluation; 4.Plan, prepare, and establish appropriate written Individual Education Plan goals and objectives; 5.Attend student's scheduled IEP meeting; 6.Administer and implement physical therapy program during 30 site visits (scheduled by the contractor) occurring from the date of contract award through June 30, 2013; 7.Re-assess and revise program, as necessary, as status of student changes; 8.Instruct and teach students, families and teachers as indicated on IEP; 9.Make referrals to other health professionals and integrate physical therapy program with other aspects of health care; 10.Confer with physician and other health practitioners to obtain information and make recommendations and revisions in treatment program; 11.Supervise and train education technicians and/or certified teachers in physical therapy assistance to be carried out when the physical therapist is not at the school; 12.Provide weekly on site services during the school year; as warranted by student need; 13.Provide a progress report for individual students' to coincide with the school's grading periods; 14.Attend a contract provider's meeting with the Agency Special Education Office; 15.Provide a copy of their state certification to the Agency Special Education Office and Kayenta Community School within two weeks of the beginning date of service; 16. Provide a comprehensive evaluation report; Evaluations reports will be comprehensive reporting results from standardized and nonstandardized assessments. The PT will determine from test results if the student will qualify for therapy and make recommendations to the educational team. 17. Attend meetings to explain assessment co parents and multi-disciplinary team; The PT will attend Multidisciplinary Team meetings as scheduling permits. Evaluation results will be explained to the parents and multidisciplinary team by the therapist. Results will be presented in language that will be easy to comprehend and related to developmental and school performance. Appropriate paperwork will be completed to qualify students. 18. Keep in contact with Special Education Teacher/Principal at Kayenta Community School to provide treatment; The principal and special education teacher will be kept informed of planned visits, cancellations, and scheduled meetings. 19. Provide Physical Therapy services individually or to small groups of identified students; The PT will provide these services on a regular schedule in cooperation with the students' teachers and other service providers. Services will be provided at Kayenta Community School two days per month. These services will be provided by a Registered Physical Therapist (PT). 'This service will be provided in accordance with the student's IEP which will specify the number of minutes per week or month and method of delivery (direct/consultative, group/individual, pull-out/inclusive). PT treatment approaches may include, but are not limited to gross motor development, range of motion, gait training, and positioning. Consultation with classroom staff and parents will also be provided. Follow-up activities would be provided to special education and/or classroom staff to reinforce goals and objectives. In-service training for school staff and parents may also be arranged. The PT will report on progress and complete daily logs for attendance. The PT will consult with appropriate team members and community agencies as needed. 20. Attend IEP meetings; The PT will attend IEP meetings as scheduling permits and complete relevant portions of the IEP including Present Level of Performance, Goals and Objectives, and Service time. Parents will be provided with a Prior Written Notice by the PT for any program changes. 21. Provide Arizona Certification and background clearance to Kayenta Community School. The PT will have Arizona Certification and federal background clearance. 2. ALL OTHER TERMS AND CONDITIONS TO REMAIN THE SAME.
