- Solicitation Cover Letter CSOSA-12-R-0041 Combined Synopsis Solicitation CSOSA-12-R-0041 The Court Services and Offenders Supervision Agency for the District of Columbia is seeking Faith Based organizations capable of delivering Faith-Based Transitional Housing Services for select offenders located in Washington D.C. The contractor will be required to provide services that will include but is not limited to: all facilities, labor, material and equipment necessary to provide drug-free Faith Based Transitional Housing (FBTH) and support services to adult male and female probationers and parolees to assist their transition back into the community. The support services shall include: daily living activities, comprehensive needs assessment, case planning, individual and group counseling, education and vocational advocacy, job placement/search, life skills training, substance abuse education, relapse prevention, discharge/aftercare planning and other essential services as required. The North American Industry Classification System (NAICS) code is 623220, Size Standard $7.0 Mil. This solicitation is open to all sources. This solicitation will result in a firm-fixed price contract. The performance period is one (1) base year and four (4) one year option periods. The Option Periods are exercised at the preference of the Government. This solicitation may result in a single or multiple award contracts. The place of performance for the resulting contract will be in the District of Columbia. Questions regarding this Request for Proposal (RFP) must be submitted in writing via e-mail or fax to the contracting officer identified for this solicitation no later than August 29, 2012 at 11:00 A.M. Eastern Standard Time (EST). You may email questions to donna.fletcher@csosa.gov or fax them to (202) 220-5711. Offerors must submit their proposal by September10, 2012 at 11:00 A.M. EST (local). Proposals received after this date and time will be considered late and will not be considered for award. The Government requests that proposals be valid for a period of up to 120 days after submission. All interested parties are responsible for monitoring the FedBizOpps website for release of solicitation updates, amendments and changes to the solicitation. No paper copies of the solicitation will be issued. Prospective Offerors must be registered in Central Contractor Registration (CCR) to be eligible for a contract award. Registration requires applicants to have a Data Universal Numbering System (DUNS) provided by Dunn & Bradstreet (D&B) and a Tax Identification Numbers (TIN)
