- This is a Combined Synopsis/Solicitation for commercial items prepared in accordance with the format in FAR Subpart 12.6, as supplemented with additional information included in this notice and in accordance with the simplified acquisition procedures authorized in FAR Part 13. THIS ANNOUNCEMENT CONSTITUTES THE ONLY SOLICIATION; QUOTES ARE BEING REQUESTED AND A WRITTEN SOLICITATION WILL NOT BE ISSUED. This solicitation is issued as a Request for Quote (RFQ) under RFQ Number VA246-12-Q-3416. All bidders must be registered on www.ccr.gov and https://orca.bpn.gov. Any amendments issued to this solicitation will ONLY be available on the FBO website (www.fbo.gov ). The solicitation document and incorporated provisions and clauses are those in effect through Federal Acquisition Circular 2005-53. This procurement is a 100% Small Business Set-Aside. The associated North American Industry Classification System (NAICS) code is 423450 with a small business size standard of 100 employees. This RFQ is a request for Wheelchair Equipment which meets the requirements within this RFQ to be delivered FOB DESTINATION to the VA Medical Center, Durham, 508 Fulton Street, Durham, NC 27705. This Combined Synopsis/Solicitation is for the purchase of the following commercial items. The following items, Brand Name or Equal to Henry Schein item numbers. If offering an equal, provide the manufacturer name, brand, part number, unit price and total cost. ITEM 0001 - Quantity of 1 Each Henry Schein No. 7765414 Mobile Compact Wheelchair Platform ITEM 0002 - Quantity of 1 Each Henry Schein No. 7764463 Universal Headrest for Mobile and FF Plat ITEM 0003 - Quantity of 1 Each Henry Schein No. 7760202 Bariatric Bench FAR 52.212-1, Instructions to Offerors -- Commercial Items (FEB 2012), applies to this acquisition. Technical and Price Evaluation will be in accordance with FAR 52.212-2 Evaluation-Commercial Items (Jan 1999). The Government will award a Firm Fixed Price contract to the responsible offeror whose offer, conforming to the requirements in this RFQ, will be most advantageous to the Government, price and other factors considered. A best value award will be made based upon the following factors: a. Technical: ability to provide products which conform to the characteristics and requirements listed in this solicitation; and b. Price In accordance with FAR 52.212-3 Offeror Representations and Certifications-Commercial Items (FEB 2012), an offeror shall complete only paragraph (b) of this provision if the offeror has completed the annual representations and certifications electronically via https://www.acquisition.gov. If an offeror has not completed the annual representations and certifications electronically at the ORCA website, the offeror shall complete only paragraphs (c) through (o) of this provision. FAR Clause 52.212-4 Contract Terms and Conditions - Commercial Items (FEB 2012) applies to this acquisition. The clause at FAR 52.212-5, Contract Terms and Conditions Required to Implement Statutes or Executive Orders - Commercial Items (FEB 2012) applies to this acquisition. The following clauses under subparagraph (b) apply: 52.204-10, 52.209-6, 52.219-4, 52.219-4, 52.219-28, 52.222-3, 52.222-19, 52.222-21, 52.222-26, 52.222-35, 52.222-36, 52.222-37, 52.223-18, 52.225-3 Alternate I, 52.225-13, 52.232-34. The following VAAR clauses are also applicable to this acquisition: 852.203-70 Commercial Advertising (Jan 2008), 852.211-73 Brand Name or Equal (Jan 2008), 852.246-70 Guarantee (Jan 2008), 852.246-71 Inspection, and 852.273-76 Electronic Invoice Submission The following VAAR Provisions apply to this acquisition: 852.273-70 Late Offers and 852.273-74 Award without exchanges.
