- THIS IS A SOURCES SOUGHT ANNOUNCEMENT ONLY. This is a sources sought notice for identification of potential vendors to provide all required Milk & Dairy products to the Nutrition and Food Services and the Veterans Canteen Service on an as needed basis at the following facilities: Clement J Zablocki VA Medical Center, Milwaukee, WI; William S Middleton VA Medical Center, Madison, WI; VA Medical Center, Tomah, WI; Oscar G Johnson VA Medical Center, Iron Mt, MI; Capt James A Lovell Federal Health Care Ctr, North Chicago, IL; Edward Hines Jr VA Medical Center, Hine, IL; Jesse Brown VA Medical Center, Chicago, IL. Delivery required daily, Monday thru Friday. An Indefinite Delivery Contract (IDC) may be issued with a delivery performance period from October 1, 2012 through September 30, 2013. The solicitation package may be available on or about August 31, 2012. The purpose of this notice is to gain knowledge of potential qualified sources and their business size classification relative to NAICS 311511. Consistent with the VA's Veterans First Contracting Program. VA is particularly interested in identifying Service-Disabled Veteran-Owned Small Businesses and /or Veteran-Owned Small Businesses with the capability to perform. Responses to the information requested will assist the Government in determining the appropriate acquisition method. All responses to this notice shall include company name, address, point of contact, phone number, DUNS number (www.dnb.com), list of available products, the geographical location the respondent is able to service, and a list of the respondent's establishment's certifications from private, State and/or local agencies documenting their safely and sanitary conditions and procedures. Responses to this notice should be sent via USPS to the attention of Sandra Maren, Department of Veterans Affairs, Great Lakes Acquisition Center (69D), 115 S 84th St, Ste 101, Milwaukee, WI 53214-1476 or email to sandra.maren@va.gov. Telephone responses will not be accepted. Responses must be received in writing no later than 10:00AM CST, Friday, August 31, 2012. All contractors interested in doing business with the Government must be registered in the Central Contractor Registration (CCR) database. The website for registration is www.ccr.gov. Service-Disabled Veteran-Owned Small Businesses and Veteran-Owned Small Businesses should also indicate whether the company is verified by the VA in the Vendor Information Pages (VIP) Database at www.vetbiz.gov.
