- J&A for Pelvic Task Trainer IAW FAR 6.302-1(a)(ii) and the Justification and Approval (J&A) for Logical Follow on Limited Source dated 7 August 2012, this is a combined synopsis and Request for Quote (RFQ). Please refer to the attached Statement of Work (SOW) for Pelvic Task Trainer, Non Haptic (PTT-NH) dated 7 August 2012, and the Standard Form SF 1449 for details. Your quote is due by 2 p.m. EST on Tuesday, 14 August 2012. Please send your quote via email to our Contract Specialist, MAJ Michael Ramsay, at michael.ramsay@us.army.mil, referring to RFQ#W900KK-12-T-0050. GENERAL REQUIRMENTS: The US Army Assistant Project Manager Medical Simulation (APM MedSim) hereby requests for a firm fixed priced (FFP) quote from your firm for eight (8) Clinical Female Pelvic Trainer Mk3 - Standard (Part #: 60900) and eight (8) Clinical Female Pelvic Trainer Mk3 - Advanced (Part #: 60905) for the Veterans Health Administration (VHA) Simulation Learning, Education and Research Network (SimLEARN) Program. The contractor shall provide the supplies/services required under Section B of this delivery order in accordance with the attached SOW dated 7 August, 2012. The Contractor shall deliver the supplies/services required under Section B of this delivery order to locations identified in section 3.5 of the SOW. The Contractor shall deliver Not Later Than (NLT) 22 September 2012. Early delivery is authorized. OTHER REQUIREMENTS Please specify the complete terms of the standard warranty, if any. All items to be delivered under this contract shall be packaged, packed and marked to prevent deterioration and damage during shipping, handling and storage to ensure safe arrival at destination. All inspection and acceptance shall be at destination by the Government. Final inspection to be performed at the device delivery site will be directed and witnessed by the technical representative of the Procuring Contracting Officer (PCO). Final acceptance of this Contract Line Item will be evidenced by signature of the PCO or his/her duly authorized representative. Please apply quantity discounts as applicable.
