- Background: Indian Health Service (IHS) Agency Health Priority 1 is to renew and strengthen its partnership with Tribes. It is the goal of the IHS Health Promotion Disease Prevention Program (HPDP) to create healthier American Indians and Alaska Native comunities by developing, coordinating, implementing and dissemination effective health promotion and chronic disease prevention programs through collaboration with key stakeholders and by building on individual, family and community strengths and assets. The purpose of this requirement is to build capacity through implementation of public health strategies. Effective strategies reduce health conditions and chronic diseases related to lifestyle issues. Offerors are requested to submit a proposal for services that will support their Indian Reservation community while supporting the goal of the HDDP program. The proposals for services are being sought in one of the following public health strategy areas: Public Health Assessment, Coalition and Collaboration Development; Training and Programs. Solicitations will be send to Tribal offices within the Phoenix Area Indian Health Service. The Phoenix Area comprises of Tribes located in the states of Arizona, Nevada and Utah. Proposals will be evaluated on criteria specified in the solicitation. Five awards will be made based on best value to the Government.
