Description
THIS IS A SOURCES SOUGHT NOTICE ONLY. THIS IS NOT A REQUEST FOR PROPOSAL. The Department of Veterans Affairs Grand Junction Veterans Affairs Medical Center (GRJVAMC), located in Grand Junction, Colorado is contemplating an Ophthalmology Healthcare Resources Indefinite Delivery Indefinite Quantity (IDIQ) Contract which shall be Full and Open Competition after Exclusion of Sources. We are performing market research to determine if there are a sufficient number of qualified sources before going Full and Open Competition Status. These sources are as follows: (1) Service Disabled Veteran Owned Small Business; (2) Veteran Owned Small Business; (3) Small Business; (4) 8a; (5) HUB Zone; Woman- Owned. The Government shall use responses to this notice to make the appropriate acquisition decision. The proposed solicitation will be issued in accordance with FAR Parts 12 and 15. This shall be a per procedure contract, in which, the contractor shall get paid at the prevailing Medicare rates for each Current Procedural Terminology (CPT) code completed. The selected Contractor shall have previous experience in ophthalmology in private practice or clinical setting. They shall provide a full range of ophthalmology services on and off GRJVAMC facility including but not limited to: consults, outpatient clinics, Traumatic Brain Injury (TBI) Exams, Compensation and Pension Exams, and operative procedures to the Grand Junction Veterans Affairs Medical Center (GRJVAMC). The Contractor shall conduct cataract surgeries at the GRJVAMC, 2121 North Ave, Grand Junction, CO 81501. The Contractor will provide all other services at their primary business location. The building must be American Disabilities Act compliant, and they shall provide all staff, equipment, and supplies required to carry out the requirements of this contract executed at the primary place of business. The Contractor will be available 24 hours a day, 7 days a week for ophthalmology emergencies. This requirement can be met by using standby personnel. A standby schedule shall be provided to the GRJVAMC no later than the last Friday of the month for the upcoming month. The Contractor and/or Standby personnel shall be able to be on the GRJVAMC site within 1 hour of notification. Contractor will be required to perform emergency surgery on site within one (1) hour of notification. The total contract period shall be one base year with one year option periods not to exceed four additional years. The total estimated amount for this per-procedure to including all options years, shall not exceed $ 4,012,488.00. In regards to the selection criteria, while price shall be a significant factor in the evaluation of offers, the final contract award shall be based on best value to include past performance. Request for Proposal VA-259-12-R-0604 will be available for download on or about November 01, 2012. No telephone requests for this solicitation will be allowed. Offers are to be received at the SAO West NCO 19 Rocky Mountain Acquisition Center, 4100 E. Mississippi Ave., Suite 900, Glendale, CO 80246. THIS IS NOT A REQUEST FOR PROPOSAL. NAICS Code 621111 is applicable to this acquisition, the size standard $10.0. Interested contractors are reminded that in accordance with FAR 4.12, prospective contractors shall complete electronic annual representations and certifications in conjunction with FAR 4.11 required registration in the Central Contractor Registration (CCR) database and/or System Acquisition Management (SAM) prior to award of a contract. The selected Contractor must provide evidence of completion of required licensure, credentials, required training, current competencies, in which, they must be credentialed and privileged at the GRJVAMC. Also, background investigations must be validated for all contractor personnel referred to perform services under this contract prior to providing direct patient care.
