- This is a combined synopsis/solicitation for commercial services prepared in accordance with the format in FAR 12.6, as supplemented with additional information included in this notice. This announcement constitutes the only solicitation; proposals are being requested, and a written solicitation will not be issued. This solicitation is issued as a Request for Quote (RFQ). The Bureau of Indian Affairs (BIA) is soliciting offers to procure dairy products for Sanostee Day School. Refrigeration is required for all diary items while in transit to the delivery location. The North American Industry Classification (NAICS) code is 424430. The solicitation is unrestricted and is solicited as open market. Offerors are required to provide a price quote to each item listed in the description on Page 3, Line Item 0001, Standard Form 1449. Failure to provide a price quote to each listed dairy product will be considered as non-responsive. Award shall be made to the offeror whose offer responds to all line items included in the solicitation. Failure to provide for any item shall render the offer nonresponsive and will be rejected. Incomplete offers will not be considered. Award will be made on an ALL or NONE BASIS. INSTRUCTIONS: 1. Offers shall include company TAX ID Number, CAGE Number (obtain at www.ccr.gov), and DUNS Number. 2. Offerors shall submit questions related to the RFQ via email no later than 2:00 PM Mountain Standard Time, August 17, 2012. 3. The due date for submission of quotes is August 20, 2012 at 2:00 PM Mountain Daylight Savings Time. The quotes should be packaged for delivery so as to permit safe and timely arrival at destination. The quote package should be mailed or hand delivered to the following address: BIA, Albuquerque Acquisition Office, Attention: Stella Bellanger, Contracting Officer, 1001 Indian School Road NW, Ste 347, Albuquerque, NM 87104. Fax proposals will NOT be accepted. Email proposals will be accepted. Email: Stella.Bellanger@bia.gov. Email submissions shall be limited to attachments compatible with Microsoft Office Word 2007 and/or files with a.pfd file extension. Email submissions shall not exceed 3MB total in size. Quotes submitted by Federal Express or Express Mail should be sent two or more days prior to the closing date. The quotes should be sent to the address shown in the above paragraph. The Offeror is solely responsible for ensuring its proposal arrives in a timely manner. An offeror that fails to meet the due date will be considered "late" in accordance with FAR 52.212-1(f). 4. Notwithstanding FAR clause 52.212-1, the price quote acceptance period is 90 days after the date set for receipt of proposals. The Offeror shall make a clear statement in the proposal cover page that the proposal is valid until this period has ended. 5. The Government is not obligated to pay any costs incurred by an offeror in the preparation and submission of a proposal in response to this RFQ. 6. Price: Complete the (attached) Standard Form 1449. 7. Representation and Certifications 52.212-3 is required to be completed. All offerors are required to be registered in the Central Contractor Registration (CCR) in order to receive a contract or purchase order from the Federal Government pursuant to FAR 52.204-7, Central Contractor Registration (Apr 2008).
