Description
This is a SOURCES SOUGHT NOTICE FOR SMALL BUSINESS ONLY. This is not a request for proposal or quotes. This document does not obligate the Government to award a contract. The purpose of this document is to provide notice of the Government s potential intent to post a future solicitation. Responses to this announcement will be used by the Government to make appropriate acquisition decisions. After the responses to this announcement are reviewed the Government may issue a Request for Proposals solicitation for mobile lithotripsy services under NAICS codes 62111. All interested offerors will have to respond to the formal solicitation announcement in order to be considered eligible for selection. The Department of Veterans Affairs Salt Lake City Health Care System, Utah is seeking sources for onsite mobile extracorporeal shock wave lithotripsy (ESWL) to patients of the Veteran s Affairs Hospital. The hospital requires a portable state of the art digital unit that can be easily moved into operating rooms. This equipment should be in the operating room ready to treat patients at least 30 minutes prior to scheduled procedures. This non-invasive technique must be able to produce a stone free rate for renal stones ranging in size from 4mm to 20mm with overall success rate of better than 80% after a single session. The success rate for stones less than 10mm in widest diameter shall be at least 80%. The machine must also be able to accommodate patients weighing up to 400 pounds. The system must also be capable of being employed with all anesthetic types including IV sedation as well as no anesthesia. The VA Hospital also requires a technician to assist the attending urologist in the operation of this device. The technician and machine must be capable of at least an average treatment rate of one patient per hour averaged over an eight hour period. Documentation to support throughout the procedure must be provided with the proposal for evaluation. The technician will have to be able to perform these services in accordance with the policies and accreditation standards imposed by the Joint Commission. These services are to be conducted once a month with an average of 3-4 patients per scheduled visit, but possibly as low as 1 patient and as high as 6. The intended contract period will be for one base year, with four one-year option periods which MAY begin on October or November of this year.. Responses to this notice shall be emailed to the attention of Pam Matthews, Department of Veterans Affairs at pamela.matthews@va.gov. Responses must be received no later than 5:00P.M. EST Friday August 21, 2012. Any information submitted by respondents to this notice is strictly voluntary. Requests for solicitation will not receive a response since a formal solicitation for these services is not yet available. There will be no reimbursement made for any costs associated with providing information in response to this announcement. Responses to this sources sought is not a request to be added to a prospective bidders list or to receive a copy of the solicitation. If the Government chooses to post a solicitation at a later time it will be posted on the fedbizops website.
