- (i) This is a combined synopsis/solicitation for commercial items in accordance with the format in FAR Subpart 12.6 as supplemented with additional information included in this notice. This announcement constitutes the only solicitation; proposals are being requested and written solicitation will not be issued. (ii) Request for Quote (RFQ) 595262 is a combined synopsis/solicitation for commercial items. This combined synopsis/solicitation is issued as a RFQ. Submit written offers only, oral offers will not be accepted. All firms or individuals responding must be registered with the Central Contractor Registration (CCR). (iii) This combined synopsis/solicitation document and incorporated provisions and clauses are those in effect through Federal Acquisition Circular 2005-60. It is the contractor's responsibility to be familiar with applicable clauses and provisions. Clauses and provisions can be found at: http://farsite.hill.af.mil/vffar1.htm (iv) This procurement is being issued as 100% Small Business Set-Aside. North American Industrial Classification Standard: 322211; Small Business Size Standard: 500 (v) Line Item (001) 6,000 boxes for processing & shipment of tree seedlings delivered to the address stated in vii below. (vi) Please see attached for the product specifications (vii) FOB-Destination for delivery to: Lucky Peak Nursery at 15169 E Highway 21 in Boise, ID 83716. (208-373-1977) All items shall be delivered on pallets and cross banded. Deliveries shall be made on regular working days (Monday through Friday) during working hours (7:30 a.m. to 3:00 p.m.), excluding federal government holidays. Delivery NLT September 1, 2012. Responsibility and Inspection: Unless otherwise specified in the contract or purchase order, the supplier is responsible for the performance of all inspection requirements and Quality Control (QC): A. A sample of five (5) boxes will be sent to the Lucky Peak Nursery prior to shipment of the total delivery, so an inspection can be made of the product for conformance with specifications. A hand made sample is sufficient. B. Inspection will be made at the delivery point and will consist of random sampling of boxes for conformance with specifications and quantity count. (viii) The Government will award a firm-fixed price contract resulting from this combined synopsis/solicitation to the responsible offeror whose offer conforming to the synopsis/solicitation will be most advantageous to the Government, price and other factors considered. Award shall be based on Best Value to the Government. Best value can include but is not limited to price, delivery date, and technical capability. (ix) The clause at FAR 52.212-4 Contract Terms and Conditions-Commercial Items, applies to this acquisition. An offeror shall complete only paragraphs (b) of this provision if the offeror has completed the annual representations and certificates electronically via https://orca.bpn.gov. If an offeror has not completed the annual representations and certifications electronically at the ORCA website, the offeror shall complete only paragraphs (c) through (o) of this provision. (xiv) A. Variations in the quantity of any item called for by this contract will not be accepted unless the variation has been caused by conditions of loading, shipping, or packing, or allowances in manufacturing processes, and then only to the extent, if any, specified in paragraph (B) below. B. The permissible variation shall apply to all items & be limited to plus or minus 5%. C. A sample of five (5) boxes will be sent to the Lucky Peak Nursery prior to shipment of the total delivery, so an inspection can be made of the product for conformance with specifications. A handmade sample is sufficient. D. Inspection will be made at the delivery point and will consist of random sampling of boxes for conformance with specifications & quantity count. (xv) Quotes must be emailed to scorum@fs.fed.us or faxed to (208) 373-4168. Quotes are required to be received no later than 16:30 MST, August 10, 2012. I. GENERAL REQUIREMENTS A. Scope Supply and deliver 6,000 100% recyclable corrugated fiberboard boxes, c-flute in accordance with the attached specifications, 32" length x 9.75" deep x 14.50" wide, delivered F.O.B. to Lucky Peak Nursery, Boise, Idaho. II. TECHNICAL SPECIFICATIONS A. Fiberboard Boxes All fiberboard boxes furnished under this specification shall be fabricated in accordance with American Society for Testing and Materials, (ASTM) Standard Practice for Fabrication of Fiberboard shipping boxes, D5118/d 1. Type: CF: Class -Domestic: Variety - SW: Grade - 275: C Flute. Boxes shall be fabricated of 275 pound test single wall fiberboard consisting of a 33 pound corrugated member between two facings. The outer facing shall weigh not less than 69 pounds per thousand square feet and the inner facing shall weigh not less than 69 pounds per thousand square feet. The facing and corrugated medium shall be bonded with a moisture resistant adhesive (WRA). It shall be corrugated with a 33-39, 3/16 inch flutes per linear foot. 2. Style: a. Full overlap top flaps. b. Full overlap bottom flaps. c. Perforated, hinged (punch out) oblong hand hold, 3.25" wide, 1.00" high, center and positioned 3.00" from top of box. 3. Inside Dimensions: a. Length - 32.00 inches b. Width - 14.50 inches c. Depth - 9.75 inches B: Additional Requirements 1. All boxes shall have a wax impregnated inside liner (100% recyclable). 2. All manufacturing joints shall be of a glue lap type or equivalent, 2" wide with a waterproof adhesive.
