Description
Solicitation Checklist Solicitation Document The Department of Defense Education Activity (DoDEA) requires Nursing Services for the period of performance from mid-August (or date of award) through 31 October 2012 with a break from 1 November 2012 to 31 March 2013. Services will resume 1 April 2013 until 5 June 2013 in support of the Special Education Program for the NY/VA/PR district located at West Point, New York. The Government will award a contract resulting from this solicitation to the responsible offeror whose offer conforming to the solicitation will be most advantageous to the Government, price and other factors considered. The Government will use the lowest price technically acceptable source selection process. The following standards shall be used to evaluate offers: Offeror must meet or exceed the technical qualifications at 3.0 of the Performance Work Statement (PWS) in order to be considered technically acceptable: a. Award will be made to the lowest-priced offer with acceptable technical proposal and past performance rating. A written notice of award or acceptance of an offer mailed or otherwise furnished to the successful offeror with in the time for acceptance specified in the offer, shall result in a binding contract without further action by either party. Before the offer's specified expiration time, the Government may accept an offer (or part of an offer), whether or not there are negotiations after its receipt, unless a written notice of withdrawal is received before award b. Technical Proposal: For the person being proposed to perform the required services provide the following: 1.) Resume: The resume of the person(s) proposed to provide the Nursing services. Resume(s) shall describe the proposed personnel's credentials and work history to include the number of years and listing the various disabilities that have been dealt with. 2.) Certification: Proof of Nursing certification 3.) Past Performance: Past performance references with current points of contact information c. Proposal shall be received no later than August 16, 2012 at 2:00PM (Eastern) Questions and/or requests for information concerning this solicitation should be submitted in writing via email, before August 10, 2012 at 10:00AM Eastern, to Melinda Davis, Contract Specialist, at email address: melinda.davis@am.dodea.edu
