- Attachment F- Section 508 Evaluation Template Attachment E - Disclosure of Lobbying Activities Attachment D - Non-Disclosure Agreement Attachment C - Past Performance Survey Attachment B – Subcontracting Plan Template Attachment A_Wage Determination 12-233-SOL-00460_BASE This is a combined synopsis/solicitation for commercial items prepared in accordance with the format in Subpart 12.6, as supplemented with additional information included in this notice. This announcement constitutes the only solicitation; proposals are being requested and a written solicitation will not be issued. The Solicitation number is 12-233-SOL-00460 and is being issued as a request for proposal (RFP). The solicitation document and incorporated provisions and clauses are those in effect through Federal Acquisition Circular 2005-58, effective May 18, 2012. Offerors should be registered in the Central Contractor Registration (CCR) database. The North American Industry Classification (NAICS) Code is 541690. The small business size standard is $14M. A Firm Fixed Price contract is anticipated. The period of performance has a base period of 12 months with four (4) 12 months option period. The Children's Bureau (CB) in the Administration for Children and Families (ACF) is interested in procuring the services of a Contractor to plan, develop, and implement a rigorous national cross-site evaluation of the grantees to be funded under the Regional Partnership Grants to Increase the Well-being of and to Improve Permanency Outcomes for Children Affected by Substance Abuse (hereafter known as the Regional Partnership Grants Program). The Contractor will also provide evaluation-related technical assistance to the grantees to improve the quality and rigor of local evaluations and performance measurement. The Contractor will evaluate the programs and activities conducted through the RPG program and analyze the extent to which the grants have been successful in addressing the needs of families with substance abuse problems who come to the attention of the child welfare system. Finally, the Contractor will conduct strategic dissemination activities and write evaluation reports and research briefs, including legislatively mandated reports to Congress. This acquisition is being conducted using Part 12 of the Federal Acquisition Regulation. The provision at 52.212-1, Instructions to Offerors-Commercial, applies to this acquisition. Provision at 52.212-2, Evaluation-Commercial Items, is applicable. Evaluation of proposals will be conducted in accordance with the criteria identified under Section 12 of the solicitation. Refer to the attached Solicitation for the specific evaluation criteria. Offerors are advised to include a completed copy of the provision at 52.212-3, Offeror Representations and Certifications-Commercial Items, with its offer. Refer to attached solicitation for further details. The clause at 52.212-4, Contract Terms and Conditions-Commercial Items, applies to this acquisition. Refer to attached solicitation for further details. The clause at 52.212-5, Contract Terms and Conditions Required To Implement Statutes or Executive Orders-Commercial Items, applies to this acquisition. Refer to attached solicitation for further details. Please read the attached solicitation carefully and prepare your proposal in accordance with the solicitation requirements. It is the Offeror's responsibility to submit all questions and proposals by the scheduled closing date and times, in accordance with the "Instructions to Offerors". Please note that the deadline for questions is August 17, 2012 by 11:00 AM Eastern Time. All questions must be submitted via email. All responses will be posted on the Federal Business Opportunities website. This RFP does not commit the Government to pay any costs for the preparation and submission of your proposal. It is also brought to your attention that the Contracting Officer is the only individual who can legally commit the Government to the expenditure of public funds in connection with the proposed requirement. The Government reserves the right to award a contract without discussions if the Contracting Officer determines that the initial offer(s) is /are providing the best value and discussions are not necessary. It is the Offeror's responsibility to monitor the FEDBIZOPPS website for the release of any amendments to this solicitation. No telephone or faxed requests for the solicitation will be accepted.
