- This is not a solicitation announcement. This is a sources sought notice for the identification of potential vendors to provide Aortic and Mitral Mechanical and Tissue Heart Valves and Annuloplasty Rings on a consignment basis to three VISN 12 facilities. Facilities are located in Hines, Illinois; Milwaukee, Wisconsin; and Madison, Wisconsin. Indefinite Delivery Contract (IDC) may be issued with a delivery performance period from October 1, 2012, through September 30, 2013 with up to four option years. The solicitation package may be available on or about August 30, 2012. The purpose of this notice is to gain knowledge of potential qualified sources and their business size classification relative to NAICS code 339113. Consistent with the VA's Veterans First Contracting Program, VA is particularly interested in identifying Service-Disabled Veteran-Owned Small Businesses and/or Veteran-Owned Small Businesses with the capability to perform. All interested and capable respondents are requested to provide the information below. All responses to this notice shall include: company name, address, point of contact, phone number, DUNS number (www.dnb.com), list of available products, the geographical location the respondent is able to service, and a list of the respondent's establishment's certifications from private, State and/or local agencies documenting their safety and sanitary conditions and procedures. Responses to this notice should be sent via US to the attention of Karen Hammen, Department of Veterans Affairs, Great Lakes Acquisition Center (69D), 115 South 84th Street, Suite 101, Milwaukee, WI 53214 or email to Karen.hammen2@va.gov phone responses will not be accepted. Responses must be received in writing no later than 10:00 AM ET, Monday August 13th, 2012. All contractors interested in doing business with the Government must be registered in the System for Award Management database. The website for registration is www.SAM.gov. Service-Disabled Veteran-Owned Small Businesses and Veteran-Owned Small Businesses should also indicate whether the company is verified by the VA in the Vendor Information Pages (VIP) Database at www.vetbiz.gov.
