- RFQ Addenda Statetment of Work for Micron Air Jet Sieve, 200-mesh sieve and 20 mesh sieve This is a combined synopsis/solicitation for commercial items prepared in accordance with the format in Subpart 12.6, as supplemented with additional information included in this notice. This announcement constitutes the only solicitation; proposals are being requested and a written solicitation will not be issued. Solicitation Number is 465-12MR-1963 is issued as a request for quotation. The solicitation document and incorporated provisions and clauses are those in effect through Federal Acquisition Circular (FAC) 2005-60 (effective July 26, 2012). The associated North American Industrial Classification System (NAICS) code for this procurement is 334516 with a small business size standard of 500 employees. The items to be acquired are 1 each micron air jet sieve, 1 each 200 mesh sieve and 1 each 20 mesh sieve in accordance with the attached specifications. Delivery is required within 30 days after receipt of order. The place of delivery and acceptance is Mine Health & Safety Academy, US Department of Labor, Mt. Hope Laboratory, 100 Bluestone Road, Mt. Hope WV 25880. All prices shall be FOB destination. The following provisions and clauses apply to this acquisition and are hereby incorporated by reference. If paragraph (j) of the provision is applicable, a written submission is required. NOTE: The website above has been removed and is being replaced with http://sam.gov. At the time of this notice, the http://sam.gov website is not functional. In the event the sam.gov website is not functional at time of quotation submission, you must provide a copy of the completed FAR 52.212-3 Representations and Certifications for Commercial items with your quotation. Destination * Invoicing Procedures * Providing Accelerated Payment to Small Business Subcontractors INSTRUCTIONS Central Contractor Registration In accordance with FAR 52.204-7, the awardee must be registered in the Central Contractor Registration (http://www.sam.gov) prior to award. Refusal to register shall forfeit award. In the event the sam.gov website is not functional at time of quotation submission, you must provide an electronic copy of the current CCR registration. Due Date for Quotations Offerors shall submit their quotations so that MSHA receives them not later than 12:00 p.m. Eastern Time on August 17, 2012. FAX quotations shall not be accepted. E-mail quotations shall be accepted at Faykus.elizabeth@dol.gov. Offerors' quotations shall not be deemed received by the Government until the quotation is entered into the e-mail address inbox set forth above. Quotations shall be sent to the Mine Safety and Health Administration, Acquisition Management Division, Attention: Elizabeth Faykus, 1301 Airport Road, Beaver, WV 25813. Because of heightened security, FED-EX, UPS, or similar delivery methods are the preferred method of delivery of quotes. If quotes are hand delivered, delivery shall be made on the actual due date and a 48 hour (excluding weekends and holidays) prior notice shall be provided to Elizabeth Faykus, Contract Specialist on 304-256-3388. Quotation Evaluation: Evaluation Factors Award preference will be given to small businesses. A written notice of award or acceptance of an offer, mailed or otherwise furnished to the successful offeror within the time for acceptance specified in the offer, shall result in a binding contract without further action by either party. Before the offer's specified expiration time, the Government may accept an offer (or part of an offer), whether or not there are negotiations after its receipt, unless a written notice of withdrawal is received before award. Non-Governmental Acquisition Support The Government may utilize non-Governmental acquisition support for processing of quotations submitted in response to this solicitation. These individual(s) will have access to the quotation information and will be assisting the Contracting Officer in the procurement process by performing services including, but not limited to, preparing the quotations for submission to the technical evaluators, performing past performance checks, and preparing award documents. The non-Governmental acquisition support will not conduct technical evaluations of any quotation and will not be involved in the final decision as to the awardee under this procurement.
