Description

PRESOLICITATION NOTICE FOR NURSING HOME. The Department of Veterans Affairs, Alexandria VA Health Care System located at 2495 Shreveport Highway, Pineville, LA 71360 has a requirement for Nursing Home Services provided to qualified Veterans Affairs beneficiaries in the Kaplan, LA area. The Alexandria VA Health Care System intends to solicit and award a competitive agreement to a Nursing Home facility within five (5) miles of the boundaries of the city of Kaplan, Louisiana. All contract details, which include full specifications and Statement of Work, will be included in the Request for Proposal (RFP). Interested parties must meet evaluation criteria and inspection standards as part of the proposal evaluation process. The Contractor shall provide Nursing Home services, to eligible Veteran beneficiaries, at the contractor's facility and shall be a Medicare/Medicaid certified facility. It is the Alexandria VA Health Care System policy not to accept offers at prices exceeding the allowable, prevailing Medicaid rates for this locale. The contracted facility is expected to accept patients as needed and provide assisted living services throughout the contract period in accordance with the Statement of Work. The facility that this notice is targeting will take qualified patients as needed and provide them with the services outlined in the agreement. The Government will award of a Basic Order Agreement (BOA) under authority of 38 U.S.C. 1720 to the successful offeror of this requirement. The performance period will be one (1) year from date of award with four (4) possible options years available for renewal, should the facility be needed beyond the initial year. The Contractor shall register in Central Contractor Registration (CCR) at the website https://www.ccr.gov and the ORCA website at https://www.orca.bpn.gov to be considered for an award. The applicable North American Industry Classification Systems (NAICS) Code for this procurement is 623110 and the Small Business Size Standard is $13.5M. Award will be made based on the best value offered to the Government. Interested parties who are capable of providing the required services and wishes to receive a copy of the RFP may submit a request in writing. A fax is also acceptable. The physical location is 2495 Shreveport Highway, Pineville, Louisiana. This requirement is subject to the availability of funds. Estimated solicitation issue date is on or about August 16, 2012 at www.fedbizops.gov. Interested parties are to submit a written proposal to the Contracting Officer no later than September 11, 2012 by 12 Noon CST. Any amendments issued to this solicitation will also be available at the URL address and offerors are responsible for obtaining any/all amendments issued.
