- THIS NOTICE IS FOR INFORMATIONAL PURPOSES ONLY. The Centers for Disease Control and Prevention (CDC), National Center on Birth Defects and Developmental Disabilities (NCBDDD) Division of Human Development and Disability contemplate award of a Firm Fixed Price Sole Source Purchase Order to GeoWise Ltd. The work shall be performed at the Contractor's facility. The Period of Performance shall be for 12 months. The North American Industrial Classification Standards (NAICS) Code is 511210 and Standard Business Size of $25M. The sole source award will be conducted in accordance with FAR Part 13.106-1(b) only one responsible source and no other supplies or services will satisfy agency requirement. The Early Hearing Detection and Intervention (EHDI) Program seeks to improve and strengthen the presentation of annual aggregate data obtained from states and territories through the use of InstantAtlas. This surveillance data application program will be used to show health trends, analyses, and statistics related to progress towards national EHDI goals (e.g., percent of infants screened for hearing loss by 1 month of age, undergo diagnostic assessment by 3 months of age, and are enrolled in appropriate early intervention programs by 6 months of age). Once the proposed Instant Atlas program is implemented, the available analyses and summary of the EHDI data will assist with the development and assessment of sound public health programs. These functions are critical to CDC's mission to promote health and quality of life by preventing and controlling disease, injury, and disability. The overall scope of this contract requires GeoWise's web design, developer, and data support to develop an EHDI National Data Dashboard with the ability to present national aggregated survey data in an improved visual "dashboard" format. This "dashboard" format will enhance the state and territorial EHDI programs' ability to make data driven programmatic decisions, inform stake holders, and improve systems procedures within their program. CDC is not requesting competitive proposals and no solicitation is available. Firms that believe they can provide the services required are encouraged to identify themselves and give written notification to the Contracting Officer no later than 2:00 PM Eastern Standard Time, by August 9, 2012. Determination not to compete this proposed action based on responses to this notice is solely within the discretion of the Government. Respondents will assume all financial costs for preparing a response to this notice. The U.S. Government assumes no financial obligation for the preparation and distribution of prepared responses.
