- Description
- The Southern Oregon Rehabilitation Center & Clinic in White City Oregon requires clinical pathology services and medical oversight for the clinical laboratory. Services shall be provided on site in the Pathology & Laboratory Medicine Service, at the Southern Oregon Rehabilitation Center & Clinics (hereinafter referred to as the SORCC), 8495 Crater Lake Hwy, Clinical Laboratory Bldg 201, White City Oregon 97503. Contractor will provide a pathologist as the Laboratory Medical Director as well as clinical and technical oversight for the SORCC laboratory for 4 to 12 hours per month. All professionals providing services under the terms of this contract will be appropriately board certified and licensed and possess the requisite skills and experience to perform the services defined in this work statement. The Contractor shall: (a) Serve as the Laboratory Medical Director, providing clinical and technical consultation as required by the College of American Pathologist's (CAP) for accreditation status. (b) Serve as the Laboratory Medical Director to meet the Clinical Laboratory Improvement Act (CLIA) requirements for the SORCC Clinical Laboratory and all associated Community Based Outpatient Clinics (CBOC) or outreach clinics. (c) Shall provide 4-12 hours of professional services per month, between 48 and 144 hours annually, onsite at the main White City facility. Specific tasks to be completed with each visit include but is not limited to: a. Formal provider consultations (hematology, chemistry and pathology) b. Provide test review/interpretation without formal consultation. c. Review selected abnormal blood smears and provide narrative interpretation. d. Review and authorize new (written) laboratory procedure and methods. e. Review laboratory Proficiency Testing. f. Review laboratory quality assurance procedures, problems and monitors. g. Review laboratory staff meeting minutes. h. Review and make recommendations regarding personnel, equipment, safety and lab reporting mechanisms. i. Review and direct laboratory efforts leading up to and subsequent to accreditation inspections. j. Within thirty days of each site visit, the contractor will send a summation report detailing activities and findings of that particular visit to the Laboratory Manager. (d) Shall be available within 2 hours when unannounced laboratory inspections occur (specifically for CAP inspections). Availability can be by phone or teleconference. (e) Shall be an ad hoc member of the Medical Executive Committee. (f) Be actively involved in the design, implementation and oversight of the quality management system in the laboratory. (g) Provide monthly review of quality control records and other quality assurance reviews as requested by the Laboratory Manager. (h) Provide availability of consultation during operational hours, either by telephone, fax, email or in person. (i) The Contractor will provide input to any Peer Review or Process Review directly relating to a patient treatment or incident resulting from actions taken as a result of the consultation.
