Description
OCSQ0266SOWTEC CONVERSION OF OSCAR's MODEL 204-BASED CLIA DATABASE TO THE QIES SERVER-BASED PLATFORM OCSQ-393-2012-0266 This is a Request for Quotation prepared in accordance with FAR Part 13, as supplemented with additional information included with this notice. THIS ANNOUNCEMENT CONSTITUTES THE ONLY SOLICITATION; A SEPARATE WRITTEN SOLICITATION WILL NOT BE ISSUED. QUOTES ARE BEING REQUESTED. The Centers for Medicare and Medicaid Services intends to award using Simplified Acquisition Procedures, a firm fixed priced purchase order (Dollar Threshold of $150,000) for "Conversion of OSCAR's Model 204-based CLIA database to the QIES server-based platform" The period of performance will be award through 1 year. The North American Industry Classification Code (NAICS) is 541611. The purpose of this purchase order is to acquire the services and expertise of a Contractor to facilitate Division of Laboratory Services (DLS) staff's completion of the tasks related to the conversion of OSCAR's Model 204-based CLIA database to the QIES server-based platform." These tasks include: performing technical review and analysis of CLIA QIES system requirements; actively participating in critical system validations and developing comprehensive process-oriented scenarios for validation purposes; and providing technical review of design documents, training plans, WebExes, E-learning modules and procedure manuals to determine completeness and accuracy. The Statement of Work/Deliverables and Technical Evaluation Criteria are attached. This Request for Quotes (RFQ) is being set aside for small business concerns. Responses must be sent electronically in MS Word format to Lucille.Lee@cms.hhs.gov no later than 11:00am on August 15, 2012. Responses shall be no more than 15 pages and shall include the following: 1) Technical Approach; 2) Key Personnel and 3) Past performance information - relevant experience to perform the required services. All responses will be evaluated on a best value basis. The government intends to award an order to the responsible offeror whose quotation offers the best combination of technical capabilities and proposed price (whole dollars); the best value (considering price and other factors). All responses/quotes submitted shall remain valid for a period of 180 days. For the purpose of establishing timely receipt of a quotation, the time will be established based the electronic submission. Interested parties must be able to affirm no conflicts of interest with parties involved in Medicare claims processing in light of the work identified in this Statement of Work. In the event that the vendor/government identifies a conflict or the appearance of a conflict, the vendor shall be responsible for providing a mitigation strategy that satisfies the government. In the event that the conflict cannot be resolved, the order cannot be awarded or an existing order may need to be terminated. Please submit proposals in whole dollar amounts only. All questions must be submitted electronically by August 7, 2011 @ 11:00am. Questions will not be answered by the telephone.
