- THIS IS NOT A SOLICITATION ANNOUNCEMENT. This is a pre-solicitation notice only. The purpose of this notice is to announce that the Dept. of Veterans Affairs (VA), Long Beach Healthcare System Facilities Management Department has a requirement to provide to procure Patient Follow-Up Services. The solicitation that will be announced shall be set-aside strictly to Small Business (SB). All interested parties must respond to that solicitation. A response to this notice is not a request to be added to a prospective bidders list or to receive a copy of the solicitation. Telephonic responses will not be entertained. Submit all responses and/or questions regarding this notice to: Joseph Mendoza Contract Officer VISN 22 Network Logistics Office 4811 Airport Plaza Drive, Suite 600 Long Beach, CA 90815-1263 Email: Joseph.mendoza2@va.gov P:562-766-2253 F: 562-961-1376 The Department of Veterans Affairs (VA), Veterans Integrated Service Network (VISN) 22, has an ongoing program to discharge outcome data related to patient's rehabilitation hospitalization for Commission on Accreditation of Rehabilitation Facilities (CARF) Accreditation, and provide quarterly data disk plus hard copy on a monthly basis to the participating VA facilities listed below. The Anticipated contract period of the performance shall be from November 1, 21012 through October 31, 2013, with the government's option to exercise four (4) one (1) year option periods. The services are to be provided to the following facilities: 1. VA San Diego Healthcare System 3350 LaJolla Village Drive San Diego, CA 92161 2. VA Long Beach Healthcare System 5901 East Seventh Street Long Beach, CA 90822 4. VA Loma Linda Healthcare System 11201 Benton Street Loma Linda, CA 92357 3. VA Greater Los Angeles Healthcare System 11301 Wilshire Boulevard Los Angeles, CA 90073 A.CONTRACTOR RESPONSISBILITIES 1.Contractor shall provide the following standard service to each participating VA Facility: a.Data collection b.Data entry c.Quarterly Disk to Facility d.Referral to Facility for identified patient concerns, problems or questions e.Standard Quarterly Report and list of unobtainable records. 2.Data will be collected by telephone interview approximately ninety (90) days following patient discharge with a minimum discharge to follow-up interval of (80) eighty days. 3.Contractor shall collect all information necessary to complete the Follow-up Coding Sheet. 4.Contractor shall make up to five (5) attempts to contact each patient/caregiver for data collection. The data source may be either the patient, a family member, paid attendant or other individual who is knowledgeable regarding the patient's functional status. Contractor uses the source for data collection that is provided by the VA. 5.Contractor shall provide an informational service only for the purposes as determined by each facility. 6. Contractor shall not be responsible for the medical interpretation of data collected, or the interpretation, use or consequences of use of this data by the VA facility. 7.Contractor agrees that all information disclosed to them is confidential in nature and that it will not disclose such information to any third party without the prior written consent of the participating VA facility Contracting Officer's Technical Representative (COTR) whose consent may be withheld, at the sole discretion of the participating VA facility. Reports will be identified by VA facility number as follows: VA Greater Los Angeles- 691, VA Long Beach- 600, VA San Diego- 664, and VA Lorna Linda- 605. Data will be released by the contractor to the participating VA facility through a designated representative of the participating VA facility. 8.Contractor's service representative (telephone interviewer) is authorized by the participating facility to conduct said telephone interviews on behalf of the participating VA facility, and will identify themselves as such to the patient/caregiver. B.VA RESPONSIBILITIES 1.The participating facility shall notify patient upon discharge that a representative will contact them ninety (90) days post-discharge for a personal interview. 2.Each participating facility shall provide the following information to the contractor, which is necessary to contact the patient and complete the follow-up record: a. patient name b.patient impairment group c.patient identification number d.patient admission and discharge date e.patient discharge destination f.patient telephone number g.Additional personal contact telephone number patient age and gender. C.REPORTS: All reports shall be shipped to the individual listed on the VA facility information form to be provided after contract award. The facility information form is to be updated annually. D.FOLLOWUP CODING SHEET: Contractor shall submit with their quote a blank sample copy of their Follow-up Coding Sheet. The Coding sheet shall, at a minimum, consist of the coding sheet used to survey the patient, and another sheet submitted to the facility to update their information. E.QUALIFICATIONS: Personnel performing contract services must be fully qualified and competent, FIM"'' 1 credentialed/trained Nurse. "Fully qualified" is defined as trained and experienced to successfully provide the contracted services as required herein. The Contractor shall provide the required contract services to the VA's Physical Medicine and Rehabilitation Service for patient follow-up assessment by meeting, at a minimum, the requirements listed below: o Functional Independence Measure Instrument (FIM) credentialed facility. o Have successfully processed FIM data/assessments according to Uniform Data System standards o Sufficient knowledge and experience to interface effectively with VA computer systems o Credentialed personnel for scoring of the Functional Independence Measure (FIM) instrument. o The Contractor shall provide written verification of the competency of their personnel and a list of credentials of approved personnel actively providing services. Documented proof shall be submitted to the Contracting Officer with the Vendor's quotation. o Contractor shall be a licensed vendor of the Uniform Data System for Medical Rehabilitation, a division of UB Foundation Activities, Inc. *** FIM"' is a trademark of the Uniform Data System for Medical Rehabilitation, a division of UB Foundation Activities, Inc. ***
