- Description
- The Portland VA Medical Center anticipates contracting for Intraoperative Neurophysiologic Monitoring Services to Portland Veterans Affairs Medical Center, (PVAMC). The provided care shall include but not be limited to evoked potentials and electromyography services normally provided to civilian health care facilities for similar cases. Clinical Neurophysiologist services are required. The Contractor is to provide supplies and equipment necessary to complete this testing. This support will be required on an as needed basis for a base period, with the option to extend the contract for an additional four (4) one year periods. The Contractor shall ensure that the staff assigned to serve in operating rooms possess a knowledge of the affects of anesthesia and of various artifacts such as electrical interference from operating room equipment, etc, in order to be able to assess their effects on the data collected during the surgical procedure. Contract staff must have knowledge of potential morbidity caused by the monitoring process itself, quality control systems and procedures for preventive maintenance and performance testing and calibration of equipment and recognize when problems are beyond scope of expertise and must be referred to others. Contract staff shall be able to effectively communicate with surgical team members in order to provide recommendations based upon interpretation of monitoring results. The Contractor shall provide Clinical Neurophysiologists to provide electrophysiological (EP) monitoring of patients undergoing spinal surgery at the PVAMC. Electrophysiological (EP) monitoring includes, standard lower and upper limb somatosensory evoked potential (SSEP), motor evoked potential (MEP) electromyography (EMG) and brainstem evoked potential (BSEP) for medically indicated neurosurgical cases, such as cervical and lumbar spinal stenosis, cervical myelopathy, and cervical and lumbar disc herniations associated with radiculopathy. The contractor shall provide Clinical Neurophysiologists to provide real-time electrodiagnostic monitoring and interpretation for patients undergoing spinal surgery at the PVAMC. This service will begin when the administration of anesthesia commences and continue until the end of the case. The Clinical Neurophysiologist will provide on-site or remote real-time intra-operative electrodiagnosis during the entire monitored neurosurgical procedure. The neuro-monitoring services described in this contract are subject to the overall technical direction of the VA Chief of Neurosurgery (or delegate). The Contractor shall provide Intraoperative Neurophysiologic Monitoring Services for VA patients for an estimated 3 per month, at an estimated 5 hours of time for each case. The operating room schedule will be established the week prior and normally be a set day each week with the cases typically between the hours of 7 a.m. and 5 p.m. Although most cases will be scheduled during normal working hours (7a.m. through 5 p.m.), the PVAMC may request emergency procedure monitoring outside of the normal tour. These services will be mutually coordinated between both parties if Contract personnel are available. Service hours after 5 p.m., weekends, and holidays can be bill at the overtime rate.
