- I.General: Need Statement: VISN 23 needs complete replacement laparoscopic surgical systems at the following medical centers: Facility VA Black Hills Health Care System - Fort Meade 113 Comanche Road Fort Meade, SD 57741 VA Central Iowa Health Care System - Des Moines 3600 30th Street Des Moines, IA 50310-5774 Fargo VA Health Care System 2101 Elm Street Fargo, ND 58102 Iowa City VA Health Care System 601 Highway 6 West Iowa City, IA 52246-2208 VA Nebraska Western Iowa Health Care System - Omaha 4101 Woolworth Avenue Omaha, NE 68105 Sioux Falls VA Health Care System 2501 W. 22nd Street Sioux Falls, SD 57117-5046 II.Scope of Service a.Black Hills VAHCS (Fort Meade) will be replacing 2 existing Laparoscopic Systems and also procuring 1 additional system for a total of 3 Laparoscopic Systems. Monitors and carts should be included with their systems. b.Central Iowa VAHCS (Des Moines) will be replacing 2 existing Laparoscopic Systems and also procuring 2 additional systems for a total of 4 Laparoscopic Systems. No monitors or carts are needed for these systems as they will be placing them on articulating arms in their Operating rooms. c.Fargo VAHCS will be replacing 2 existing Laparoscopic Systems and also procuring 2 additional systems for a total of 4 Laparoscopic Systems. No monitors or carts are needed for these systems as they will be placing them on articulating arms in their Operating rooms. d.Iowa City VAHCS will be replacing 4 existing Laparoscopic Systems and also procuring 2 additional systems for a total of 6 Laparoscopic Systems. No monitors or carts are needed for these systems as they will be placing them on articulating arms in their Operating rooms. e.NWI VAHCS (Omaha) will be procuring 1 additional system for a total of 1 Laparoscopic System. Monitors and carts should be included with their system. f.Sioux Falls VAHCS will be replacing 3 existing Laparoscopic Systems for a total of 3 Laparoscopic Systems. Monitors and carts should be included with their systems. III.Equipment Requirements a.VISN 23 requires complete surgical systems with universal capability to perform bronchoscopy, laparoscopy, arthroscopy, Urology, ENT, GI, and General procedures to ensure flexibility of OR suites. The systems must be backward compatible with prior technology to enable use of currently owned scopes, and cameras that are not being replaced. Processors as well as cameras must be high-definition. b.The Equipment needed includes, but is not limited to the following items: i.HD Video Processors ii.Universal, CF Compatible light sources with autoclavable cables iii.Light guides and cables iv.HD LCD Monitors, both 26" and 15" v.Insufflation Units vi.HD Printers vii.Video Procedure Carts viii.Workstations ix.Rollstands x.Tray sets a.Quarter size arthroscopic tray b.Laparoscopic tray c.Necessary trays, containers, and lids for other equipment xi.Arthroscopic Telescopes a.0, 30 and 70 degrees b.Autoclavable c.Wide-angle field of view d.4x160mm and 2.7x160mm working lengths xii.Telescope Sheaths a.For 4x160mm and 2.7x160mm telescopes xiii.Trocar Spikes, blunt a.4x160mm and 2.7x160mm working lengths xiv.Laparoscopes a.0, 30, and 45 degrees b.autoclavable c.Full screen, 5.4x300mm working length d.Full screen, HD, 10x310mm working length e.Full screen, 10x460mm working length xv.Bronchoscopes a.2.8mm diameter, 1.2mm channel b.60cm working length c.90 degree field of view, and angulation of 180degree/130 degree (up/down) xvi.Tracheal Intubation Fiberscope xvii.Camera Heads a.E type and Q type b.Autoclavable c.High definition d.1.2x, eye piece type IV.Trade In of Equipment a.In order to offset the cost of this procurement each Medical Center is offering to trade in various items from their existing equipment that are being replaced by this new equipment. b.The list of items for trade in is contained on a separate document. c.Vendors should include cost reduction due to equipment traded as separate line items on their bid. V.Required Training Services a.The Vendor shall provide all labor, including travel, per diem, materials, non-consumable supplies, transportation, equipment, consultation and every other item of expense necessary to provide training services for all participating VA Medical Centers on the new laparoscopic surgical systems and equipment. b.The Vendor will provide the medical staff at all participating VA Medical Centers with In-Service Training on a quarterly basis. The Vendor will provide a yearly schedule for the accomplishment of quarterly In-Service Training. The Vendor agrees to provide training manuals and/or online training documentation to be used as a reference. c.The Vendor will also provide access to a 24 hour hotline phone consultation at no charge to VA Medical Centers in addition to the initial training and written manuals and/or online training documentation. d.The Vendor will provide training for Biomedical Engineering staff and Sterile Processing Staff, as well as provide detailed, technical manuals.
