- SOW Wage Rate and Past Performance worksheet Waste Collection and Removal. This is a combined synopsis/solicitation announcement, proposals are being requested and a written solicitation will not be issued. The USDA Forest Service intends to award a Firm-fixed-price contract for the following project: A detailed description of requirements is included in the attached statement of work. This solicitation is issued as a Request For Proposal (RFP). Applicable clauses and provisions and wage determinations are attached. (1) The offeror must complete the Representations and Certifications on line at http://orca.bpn.gov. (2) This combined synopsis/solicitation is being issued as a Total Small Business Set-Aside. The NAICS code for this requirement is 562111, and the small business size standard is $12.5 million. (3) The solicitation document and incorporated provisions and clauses are those in effect through Federal Acquisition Circular 2005-33. (4) The only site visit is scheduled for 9:00 a.m. Central Standard Time (CST) on Tuesday, August 7th, 2012, at 100 Van Morgan Drive, Golden Pond, KY 42211-9001. Site visit is not mandatory. (5) Period of performance is Base with 4 Option Years. (6) FAR 52.212-1, Instructions to Offerors-Commercial items (June 2008), applies to this acquisition. In addition to written price quotes, offerors are instructed to submit 3references including contact name and number for similar work completed as well as a description of the equipment intended for use on this project. (7) Evaluation factors. Past performance and technical capability is greater than the price. (8) Questions regarding this solicitation shall be e-mailed to Robert R. Sigler at rsigler@fs.fed.us. All questions regarding this solicitation shall be received electronically no later than the close of business on Monday, August 13th, 2012. Answers will be provided by the 4 PM on Friday, August 17th, 2012. (9) Proposals shall be received no later than Tuesday, August 22, 2012 at 12 noon CST addressed to Robert R. Sigler, 100 Van Morgan Drive, Golden Pond, KY 42211. NOTE: The United States Postal Service only delivers mail to our office on Tuesday and Friday of each week. Electronic proposals are preferred. E-mail rsigler@fs.fed.us. (10) The Contractor must be insured IAW FAR PART 28.303, a copy of the Certificate of Liability Insurance is required at time of Award. Any amendment(s) issued to this RFP will be published on this website. Therefore, it is the Offeror's responsibility to visit this website frequently for updates on this procurement. (11) Proposals must be valid for sixty (60) days after due date. (12) Proposals shall encompass the entire project to include submittals.
