- This is a combined synopsis/solicitation for commercial items prepared in accordance with the format in Subpart 12.6, and 13, as supplemented with additional information included in this notice. This announcement constitutes the only solicitation; proposals are being requested and a written solicitation will not be issued. (II) The provisions and clauses incorporated herein are those in effect through Federal Acquisition Circular 2005-23, effective December 26, 2007. This requirement is being synopsized as sole source under standard industrial classification code 8062 The Department of Veterans Affairs Medical Center/Employee Education Resource Center, St. Louis, MO proposes to enter into a non-competitive commercial item contract with Institute for Healthcare Improvement., 20 University RD STE 7 Cambridge, MA 02138 to sell their rights to broadcast live via VAKN to all the medical centers."The 24th Annual IHI National Forum": (1) This solicitation is unrestricted and The North American Industry Classification System (NAICS) is 541990. The software licenses are proprietary only to the company. Interested persons may identify their interest and capability to respond to the requirement or submit proposals by 31 July 1:00pm cst to Wanakee Strickland, wanakee.strickland@va.gov,. Questions can also be permitted no later than Thursday July 26, 2012 @ 1:00 pm CST. The following provisions and clauses shall apply to this solicitation: 52.252-2 Clauses Incorporated by Reference. This contract incorporates one or more clauses by reference, with the same force and effect as if they were given in full text. Upon request, the Contracting Officer will make their full text available. Offerors must include a completed copy of the FAR 52.212-3 Offeror Representations and Certification along with his/her proposal; FAR 52.212-4 Contract Terms and Conditions Commercial Items; 52.212-5 Contract Terms and Conditions Required to Implement Statutes on Executive Orders Commercial Items and in accordance with FAR 12.603(c) (2)(xii) the following provisions under the clause apply (b)(11)-(b)(15), (b)(19)(i), (b)(21), (b)(25) (b)(26) applies if Government Purchase Card is used, (c)(1), (c)(2), and VAAR 852.270-4 Commercial Advertising; 852.237-70 Contractor Responsibilities; and 852.270-1 Representatives of Contracting Officers. 1. Title of Project: "24th Annual Institute for Health Care Improvement (IHI) Forum" Satellite Broadcast and rebroadcasting of the Annual IHI National Forum on Quality Improvement in Healthcare setting. 2.0. Scope of Work: The Department of Veterans Affairs (VA), Veterans Health Administration (VHA) is seeking vendor to provide a live viewing uplink satellite feed, rebroadcasting, close captioning, for the 24th Annual Institute for Health Care Improvement conference. The IHI conference will be held in Orlando Florida on December 11 - 12, 2012. Contractor shall broadcast live to the entire VHA via the VA Knowledge Network (VAKN) satellite system. Live broadcast will occur on the above dates, with the VAKN rebroadcast dates and times to be determined by VHA's Employee Education System (EES) in collaboration with the contractor. " IHI will provide detailed information regarding specific details pertaining to the scheduling of speakers for purposes of VAKN broadcasting times. All Forum content will reside on the VAKN. "IHI will provide VA with the satellite coordinates necessary to receive Forum/conference broadcast and the necessary authorization to receive them. The Government is responsible to ensure that the broadcast/rebroadcasts shall be compliant with Section 508 Standard 1194.24 Video or Multimedia Products. "IHI will provide EES with data on VA participation as determined from the pre-program planning work and design a website registration process and mechanism to receive continuing education credits for IHI. 2.3. The contractor, will provide VA with the satellite coordinates necessary to receive all conference broadcasts and the necessary authorization to receive them. In addition to the live broadcast, the contractor shall provide the Government the rebroadcasting rights. The government will be responsible to record and ensure that the broadcast are compliant with Section 508 Standard 1194.24 Video or Multimedia Products. 2.4. The contractor will provide EES with data on VA participation as determined from the pre-program planning work and design a website registration process and mechanism to receive continuing education credits. 3.0 Background: Employee Education System (EES) Services is the largest VA provider for employee education satellite services. The use of a satellite system enables a much larger percentage of the VHA workforce to receive professional training on a regular basis. The Institute for Health Care Improvement (IHI) National Forum on Quality Improvement in Health Care is one of the premier conferences on healthcare related quality improvement in the world. Due to travel dollar funding limitations, providing this conference content via satellite broadcast provides the largest number of VHA staff to receive this high quality conference content. In addition, conference registration fees and personnel availability to attend the live face to face conference preclude large numbers of VHA staff from attending. Providing this satellite broadcast and its recorded rebroadcasts provide an economically feasible mechanism for VHA staff to participate in this content 24/7/365. 4.0 Period of Performance: The period of performance shall be from the date of contract award through the 28th of February 2013. 4.1. Government Recognized Holidays. Unless specifically authorized in writing by the Contracting Officer, no services will be provided and no charges will be incurred and/or billed to any order on this contract on any of the Federal Holidays listed below. New Years Day Labor Day Martin Luther King DayColumbus Day Presidents' DayVeterans' Day Memorial DayThanksgiving Day Independence DayChristmas Day 5.0 Place of Performance: Work shall be performed at the contractor's site. 6.0 Type of Contract: The Government anticipates award of a Firm Fixed-Price contract. 7.0 Contract Pre-Performance Meetings: The contractor shall participate in a pre-conference/forum meeting prior to the December IHI Forum. The contractor shall also participate in regularly scheduled planning teleconference meetings throughout the performance of the contract. 8.0. General Requirements: The contractor shall provide a live satellite feed and shall allow the government the digital recording/rebroadcasting rights to the 24th Annual IHI Forum conference. Task 1: Contractor shall provide the government the "24th Annual IHI Health Care Improvement Forum" live satellite feed and the digital recording rights. The recorded version shall be made available on a regularly scheduled basis via the VAKN through the 30th of April 2013. Both the live and the re-broadcasts will be aired over the VA Knowledge Network (VAKN) for the nationwide VHA facilities usage. In addition, the contractor shall provide all applicable educational handouts for the program. Deliverable 1: Contractor shall provide EES Project Manager within a minimum of 30 days prior to live broadcast, an electronic copy of the Forum handouts, Forum Agenda, promotional poster/marketing and other required supporting material for posting to VA Learning Catalog. Deliverable 2: The contractor shall provide VA with the satellite coordinates necessary to receive IHI Forum broadcast and the necessary authorization to receive them by the 1st of November 2012. Contractor shall provide rebroadcast rights to the December 11th and 12th sessions NLT 1st of November 2012. The Government will ensure that the rebroadcasts shall be compliant with Section 508 Standard 1194.24 Video or Multimedia Products. This program is simultaneously made 508 compliant with closed captioning done by the EES. The VA has the rights to rebroadcast as the recordings as often as necessary until March 2013. Deliverable 3: Contractor shall provide VA with complete titles of Quality Forum presentations being shown on December 9th - 12th, 2012 (not later than six weeks prior) so that VA participants can have the ability to schedule time away from patient care to view the broadcasts. Task 2 The contractor shall provide government with an online registration mechanism as well as for ability to provide participants continuing education credits. Specific registration data for VA participants will be determined from the pre-program planning work and design. Contractor shall work with EES staff and design web registration and support materials that will collect information from VA participants that will be used for pre and post program user data, evaluation data, and participant accreditation data. Deliverable 4: Contractor shall provide participant usage reports to the Program Manager for live broadcast VHA participation. Report shall include VHA facility location, city, state, total number of registered viewers. Contractor shall provide the "usage/participation report" no later than the 28th day of the February 2013. Electronic report shall be completed using a MicroSoft Office software program (Word, Excel etc.) Deliverable 5: Contractor shall provide VA with access to website for VA participants to print out evaluation reports and certificates for continuing education credit post program. Contractor shall also provide a phone number for a Help Desk to address technical problems with the completion of evaluation reports and obtaining continuing education certificates. Deliverable 6: The contractor will provide EES with the post program accreditation data, and numbers and demographic data on VA participants. This data will be provided by February 28th, 2012. Accreditation data shall be completed using a MicroSoft Office software program (Word, Excel etc.). 9.0. Quality Control: The contractor shall develop and maintain an effective quality control program to ensure services are performed in accordance with this PWS. The contractor shall develop and implement procedures to identify, prevent, and ensure non-recurrence of defective services. The contractor's quality control program is the means that the work complies with the requirement of the contract. As a minimum, the contractor shall develop quality control procedures that address the areas identified in the attached Quality Assurance Surveillance Plan (QASP). 10. Formal Acceptance or Rejection of Deliverables: The Government will review each deliverable and provide feedback/comments. The contractor shall have five business days to incorporate feedback/comments and make appropriate revisions. The contractor shall provide the revised version of each deliverable to the Contracting Officer's Representative (COR). The COR will review and determine final acceptance by the Government. The COR will notify the contractor of final acceptance within five business days. 11. Changes to the PWS: Any changes to this PWS shall be authorized and approved only through written correspondence from the Contracting Officer. A copy of each change will be kept in a project folder along with all other products of the project. Costs incurred by the contractor through the actions of parties other than the Contracting Officer shall be borne by the contractor. 12. Contractor Reporting Requirements: 12.1. The contractor shall submit a monthly progress report addressing the status of all active efforts. The monthly reports are due on the fifth day of each month. The reports will cover progress made for the prior month. At the conclusion of each task, the contractor shall provide a written memo documenting task completion. 12.2. The contractor shall take minutes of all formal conference calls and/or meetings held with the Project Manager. Copies of these minutes shall be attached to the next monthly progress report. 12.3. Monthly progress reports shall contain, but are not limited to, the following: 12.3.1. Status summary 12.3.2. Change request status (new, open, closed since last report) 12.3.3. Schedule status 12.3.4. The contractor shall notify the Project Manager, COR and Contracting Officer, in writing, if problems arise that could adversely impact the performance of the order. 13. Travel: None anticipated 14. Contractor Experience Requirements - N/A 15. Confidentiality and Disclosure: 15.1. It is agreed that: The preliminary and final deliverables, and all associated working papers, application source code, and other material deemed relevant by VA which have been generated by the contractor in the performance of this contract, are the exclusive property of the U.S. Government and shall be submitted to the Contracting Officer at the conclusion of the contract. 15.2 The Contracting Officer will be the sole authorized official to release, verbally or in writing, any data, draft deliverables, final deliverables, or any other written or printed materials pertaining to this contract. No information shall be released by the contractor. Any request for information relating to this contract, presented to the contractor, shall be submitted to the Contracting Officer for response. 15.3 Press releases, marketing material, or any other printed or electronic documentation related to this project, shall not be publicized without the written approval of the Contracting Officer. 16. Contractor Security Requirements: Contractor will not have access to VA Information Technology (IT) systems nor access to sensitive/personnel identifiable information (PII)
