- Description for Presolicitation Notice for VA256-12-R-0780 Ambulance Transport Services (Sources Sought Notice was released under VA25611RP0730) The VA Gulf Coast Veterans Healthcare System (VA), 400 Veterans Ave, Biloxi, MS has a requirement for ambulance transport services to eligible VA beneficiaries for the Joint Ambulatory Care Center (JACC), Pensacola, FL to include surrounding VA clinics and contracted facilities in the Florida Panhandle area which fall within the boundaries of the following counties: Escambia, Santa Rosa, Okaloosa, Walton, Holmes, Washington, and Bay. These services are to include non-emergency, Basic Life Support (BLS), and Advanced Life Support (ALS) transports both locally and long distance, farthest to include Houston, TX, Memphis, TN, and Tampa, FL to name a few. This does not include 911 services which are provided by the County. It is anticipated that this will be a firm-fixed price requirements contract with single award for a base period of one-year from October 1, 2012 through September 30, 2013 with four (4) one-year option periods to be exercised at the Government's discretion. The NAICS Code 621910 applies and the Size Standard is $7 million. There is no set-aside. The RFP (Request for Proposal) will be available on www.fedbizopps.gov under Department of Veterans Affairs on or around July 25, 2012. Potential offerors are required to use this web site to obtain a copy of the RFP in lieu of requesting a paper copy. Any and all questions are asked to await the actual release of the solicitation and shall be emailed to Melanie.Grigsby@va.gov with the following used in the subject line "VA256-12-R-0780 Ambulance Transport". Proposals shall be submitted by August 15th. Questions will be accepted up until August 7th. By submitting a proposal, the Offeror acknowledges the Government requirement to be registered in the Central Contractor Registration (CCR) database prior to award of any contract. Information about CCR registration may be found at www.ccr.gov. All responsible sources may submit a proposal which shall be considered. Evaluation preference points will be given to SDVOSB and VOSB if in competitive range. Award will be made on an all or none basis. Because the shortness of response time given please begin to assemble technical package. Below are some listed items that the offeror may be asked to provide: 1.Provide copy of accreditation from the Commission on Accreditation of Ambulance Services (CAAS). 2.Describe your organization and experience including how many years providing ambulance services. Presently we are under the assumption that there are no subcontracting opportunities; however, if there are please include any information concerning any potential subcontractors. 3.Provide evidence of your business solvency. Include the same information for any potential subcontractors. Reference from financial institution is acceptable. 4.Provide a copy of Florida license for ambulance services. 5.Please provide copies of current liability insurance. 6.Provide listing of key personnel who will be assigned to complete the requirements to include but not limited to job title, experience, educational/training background, licenses and certifications, copy of most current TB testing, copy of Heb B vaccination, and record of past accomplishments appropriate to the requirements of the solicitation. Also, certifying that no employees or subcontractors are listed on either of these websites https://www.epls.gov/ and http://exclusions.oig.hhs.gov/. 7.Provide a detailed written description of the vehicles owned by or available to you to perform under this contract to include insurance coverage. Include make of vehicle, model, and year including the location and telephone numbers of establishment where calls are received and vehicles are immediately available for dispatch. 8.Provide a copy of your Quality Assurance Plan including but not limited to training programs and performance reviews. 9.SDVOSB or VOSB Offerors must be registered in VetBiz vendor information pages, website as follows: http://www.vetbiz.gov/. Must show current certification seal to be considered, no exceptions. Fast Track may or may not be considered. 10.Offeror is responsible for Representations and Certifications at FAR 52.212-3 or noting that offeror is registered on Online Representations and Certifications Application (ORCA) at http://orca.bpn.gov. 11.Offerors are to be registered into Central Contractor Registration (CCR) database at http://www.ccr.gov prior to submitting their offer. Award cannot be made until the contractor has registered.
