Description
This is a combined synopsis/solicitation for commercial items prepared in accordance with the format in Subpart 12.6, as supplemented with additional information included in this notice. This announcement constitutes the only solicitation; proposals are being requested and a written solicitation will not be issued. This is a combined synopsis/solicitation for commercial items prepared in accordance with the format in FAR Subpart 12.6 using Simplified Acquisition Procedures for commercial information found at FAR Part 13 and as supplemented with the additional information included in this notice. This announcement constitutes the only solicitation; quotes are being requested and a written solicitation will not be issued. The solicitation number is W912PF-12-T-0111 and this solicitation is issued as a request for quote (RFQ). The solicitation is unrestricted and all interested vendors shall submit written quotes (oral quotes will not be accepted). The North American Industry Classification System (NAICS) code for this project is 561730. The purpose of this combined synopsis/solicitation is to perform the following service: "Tree cutting in emergency situations and replanting, USAG-LI, Italy" Vendors are required to indicate the total cost including: 1-Cut of trees less than 17 mt high, including removal of stump, replanting of one new tree with 4-year warranty (safety cost included), as per attached Performance Work Statement, Para. 1.6 Quantity 2, Lump Sum 2-Cut of trees over to 17 mt high, including removal of stump, replanting of one new tree with 4-year warranty (safety cost included), as per attached Performance Work Statement, Para. 1.6 Quantity 15, Lump Sum 3-Dangerous branches cutting (safety cost included), as per attached Performance Work Statement, Para. 1.6 Quantity 47, Lump Sum 4-Cut of dangerous branches after duty hrs or during holidays (safety cost included), as per attached Performance Work Statement, Para. 1.6 Quantity 7, Lump Sum 5-Removal of various dimensions trees, fallen to the ground, including removal of stump and ground refilling (safety cost included), as per attached Performance Work Statement, Para. 1.6 Quantity 3, Lump Sum 6-Removal of various dimensions trees, fallen to the ground after normal working hours or during holidays, including removal of stump and ground refilling (safety cost included), as per attached Performance Work Statement, Para. 1.6 Quantity 3, Lump Sum Contract Manpower Reporting: The Contractor is required to report all Contractor manpower (to include subcontractor manpower) required for the performance of this contract. The Contractor is required to completely fill in all the required fields in the reporting system using the following web address: https://cmra.army.mil/. The requiring activity will assist the Contractor with the reporting requirement as necessary. The Contractor may enter reports at any time during the reporting period, which is defined as the period of performance not to exceed 12 months ending 30 September of each Government fiscal year. Reporting must be completed no later than 31 Oct for every year or part of a year for which the contract is in place. Failure to comply with this reporting requirement will result in contract termination. Period of Performance: 29 working days from the award date. To be awarded a government contract you MUST have: 1. DUNS number (Dun & Bradstreet; tel: 02-28-455-379); 2. NCAGE number (www.dlis.dla.mil/Forms/Form_AC135.asp); and be registered in the Central Contractor Registration (CCR) (https://www.bpn.gov). We will discuss this at the site visit. SITE VISIT: The site visit will be held on August 2, 2012, 9:00AM. MEETING POINT FOR THE SITE VISIT: Gate 27, Depot Area, Camp Darby Military Community, Via Aurelia Tombolo (PISA), 56018 Italy HOW TO PARTICIPATE: In order to participate in the site visit all interested parties are invited to indicate their interest by submitting a written request not later than Close of Business (COB) July 24, 2012 to the following e-mail address: elisa.orsini.ln.asc@mail.mil. Please indicate the name of the person(s) (maximum of 2) who will be present at the site visit: first name, last name, date of birth, place of birth, and type and number of identification document. Your information will allow the entrance of your representative. Only people who have provided their personal data will be allowed to attend the site visit. The Guards will check the identification card at the Gate. The offerors not present on the list will be denied entrance to the Base. The offeror is solely responsible for the performance of all inspection requirements and quality control. The following provisions and clauses are applicable to this procurement: FAR 52.212-1, Instructions to Offerors - Commercial Items. The government reserves the right to evaluate quotes and award a contract without discussions with offerors. Offers shall be evaluated in accordance with FAR 52.212-2 - Evaluation - Commercial Items - The addendum to paragraph (a) is as follows: The Government will award a contract resulting from this solicitation to the responsible offeror whose offer conforming to the solicitation will be most advantageous to the Government. The following factors shall be used to evaluate offers: 1-Price: Price will be evaluated for fair and reasonableness and for unbalanced pricing. 2-Past Performance: This factor will be rated acceptable or unacceptable based on whether the past performance submitted for consideration is relevant and demonstrates successful past performance by the vendor, such as: no terminations for default, no ratings less than satisfactory by its customers/references, whether the quality of its work was satisfactory and completed in a timely manner and whether the vendor demonstrated reasonableness and cooperation during performance of the contracts. Past failure to apply sufficient tenacity and perseverance to perform acceptably is strong evidence of unacceptable past performance. In addition, failure to meet the quality requirements of a contract is a significant factor to consider in determining unsatisfactory performance. Past Performance Evaluation Ratings: Acceptable- Based on the vendor's performance record, the Government has a reasonable expectation that the vendor will successfully perform the required effort, or the vendor's performance is unknown. Unacceptable- Based on the vendor's performance record, the Government has no reasonable expectation that the vendor will be able to successfully perform the required effort. For Past Performance to be considered relevant, it must be similar in work, as described in Para. 1.6 of the attached Performance Work Statement, must be accomplished in the last three years from the date of this solicitation and the magnitude must be over $25,000.00.In the case of a quote without a record of relevant past performance or for whom information on past performance is not available or so sparse that no meaningful past performance rating can be reasonably assigned, the vendor shall be determined to have unknown past performance. Unknown shall be considered acceptable. The Past Performance is considered to be not relevant if it is not similar in work, as described in Para. 1.6 of the attached Performance Work Statement, if it is not accomplished in the last three years from the date of this solicitation and if the magnitude is under $25,000.00.The Government will conduct a past performance assessment on all contracts submitted by the vendor on the attached Past Performance Questionnaire. The Government will consider the currency and relevance of the information, source of the information, context of the data, and general trends in contractor's performance. The Government will consider the Past Performance information, as well as information obtained from any other sources. Regarding the quotes submitted in Euro currency, The United States Government is exempt from payment of Imposta Valore Aggiunto (IVA) tax in accordance with DFAR 252.229-7003, Tax Exemptions, and with Article 72 of the Italian IVA implementing Decree Law 633, dated October 26, 1972.
