- COMBINED SYNOPSIS/ SOLICITATION FOR COMMERCIAL ITEMS (i) This is a combined synopsis/solicitation for commercial items prepared in accordance with the format in Subpart 12.6, as supplemented with additional information included in this notice. This announcement constitutes the only solicitation. Proposals are requested and a written solicitation will not be issued. (ii) This solicitation/synopsis reference number is FA2517-12-T-6049 and is being issued as a Request for Quote (RFQ). (iii) This solicitation document and incorporated provisions and clauses are those in effect through Federal Acquisition Circular 2005-59. (iv) This acquisition is 100% set-aside for small businesses. The associated NAICS code is 333292 with a 500employee size standard. (v) Contractors shall submit a quote for a firm fixed price CLIN: BABY LOCK 10 NEEDLE ENTERPRISE EMBROIDERY MACHINE "OR EQUAL"; HANDS ON TRAINING; EMBROIDERY MACHINE STAND (see min technical specifications in (vi)) (vi) Description of requirements for the items to be acquired: • One (1) Baby Lock 10 Needle Enterprise Embroidery Machine "or equal" with minimum technical specifications: - Min 10 needles w/1 position for each needle - Min 5 LED lights with dimming setting - Min 1.7MB Built in memory - Push button, automatic needle threaders - Upper threads breakage sensors - HD LCD color touch screen measure min 8.5" diagonally - Capability to scan or copy logo/artwork to be reproduced - Min 3 USB drives, 2 USB ports a CD-Rom drive, 1 USB Cable direct connect to PC - Embroidery hoops: 7-7/8" x 14" (200mm x 360mm) 5-1/8" x 7-1/8" (130mm x 180mm) 4" x 4" (100mm x 100mm) 1-1/2" x 2-3/8" (40mm x 60mm) at the min to be included - Palette 9 or equal-Software capable of creating and editing embroidery designs from a variety of layout features and fonts already loaded • Hands on training for 18 people on the machine/software on site • One (1) Embroidery machine stand (vii) Delivery/installation/training shall be completed FOB Destination (Peterson AFB, Colorado 80914) within 15 days after delivery of machine. The Government will award a contract resulting from this solicitation to the responsible offeror whose offer conforming to the solicitation will be most advantageous to the government, price and other factors considered. The following factors shall be used to evaluate offers in the following relative order of importance: 1. Technical capability of the item offered to meet the Government requirement. 2. Price - The Government will evaluate the total price of the offer for award purposes. Technical and performance factors, when combined, are significantly more important than price. If offering an equal product please provide with your quote documentation showing the "or equal" item can meet the salient physical, functional, or performance characteristic specified in this solicitation. Clearly identify the item by (i) Brand name, if any; and (ii) Make or model number; Include descriptive literature such as illustrations, drawings, or a clear reference to previously furnished descriptive data or information available to the Contracting Officer; and clearly describe any modification the offeror plans to make in a product to make it conform to the solicitation requirements. REPRESENTATION BY CORPORATIONS REGARDING AN UPDAID DELINQUENT TAX LIABILITY OR A FELONY CONVICTION UNDER ANY FEDERAL LAW (DEVIATION 2012-O0004) (JAN 2012) (a) In accordance with sections 8124 and 8125 of Division A of the Consolidated Appropriations Act, 2012, (Pub.
