- THIS IS A MODIFICATION TO REQUEST FOR INFORMATION (RFI) VA240C-12-I-0015 ISSUED ON 04-09-2012. THIS IS NOT A REQUEST FOR PROPOSAL OR QUOTE. THE DEPARTMENT OF VETERANS AFFAIRS IS NOT SEEKING PRICING OR OFFERS OF ANY KIND. In accordance with FAR 15.201(e), under Exchanges with Industry before Receipt of Proposals, responses to this notice are not offers and cannot be accepted by the Government to form a binding contract. Responders are solely responsible for all expenses associated with responding to this RFI. 1. The Department of Veterans Affairs (VA) is seeking potential sources capable of providing Medical Gas services within Veterans Integrated Service Network (VISN) 10, 11, 12, 15 & 23. The gases to be supplied include, but are not limited to compressed Medical Air USP, Oxygen USP, Carbon Dioxide USP, Nitrogen NF, Nitrous Oxide USP, Helium USP, Liquid Nitrogen and specialty gas mixtures (estimate quantities are on an attached document). It is the Government's desire to award one (1) contract for the entire coverage area. 2. Interested parties would be expected to provide all personnel, equipment, tools, materials, supervision, facilities, transportation, and other items and services necessary to perform this requirement. 3. The following is a list of Hospitals and Medical Centers in each VISN: a. VISN 10 MEDICAL CENTERS: Contractor shall service the following hospitals and Medical Centers in VISN 10: Chalmers P. Wylie Ambulatory Care Center, Columbus, OH; Chillicothe VA Medical Center, Chillicothe, OH; Cincinnati, OH; Dayton VA Medical Center, Dayton, OH, Louis Stokes VA Medical Center, Cleveland, OH. b.VISN 11 MEDICAL CENTERS: Contractor shall service the following hospitals and Medical Centers in VISN 11: Alada E. Lutz VA Medical Center, Saginaw, MI; Battle Creek VA Medical Center, Battle Creek, MI; John D. Dingell VA Medical Center, Detroit, MI; Richard L. Roudebush VA Medical Center, Indianapolis, IN; Ann Arbor VA Medical Center, Ann Arbor, MI; Illiana VA Medical Center, Adanville, IL; Northern Indiana VA Medical Center, Marion, IL; Northern Indiana VA Medical Center, Fort Wayne, IN. c.VISN 12 MEDICAL CENTERS: Contractor shall service the following hospitals and Medical Centers in VISN 12: Captain James A. Lowell VA Medical Center, North Chicago, IL; Clement J. Jablocki VA Medical Center, Milwaukee, WI; Edward Hines VA Hospital, Hines, IL; Jesse Brown VA Medical Center, Chicago, IL; Oscar G. Truman Memorial Veteran's Hospital, Columbia, MO; Kansas VA Medical Center, Kansas City, MO; John J. Pershing Veteran's Hospital, Poplar Bluff, MO; Jefferson Barracks Division, St. Louis, MO; John Cochran Division, St. Louis, MO; Dwight D. Eisenhower VA Medical Center, Leavenworth, KS; Colmery - O'Neil VA Medical Center, Topeka, KS; Robert J. Dole Medical Center, Wichita, KS; Marion VA Medical Center, Marion, IL e.VISN 23 MEDICAL CENTERS: Contractor shall service the following hospitals and Medical Centers in VISN 23: Fargo VA Healthcare System, Fargo, ND; Iowa City VA Healthcare System, Iowa City, IA; Minneapolis VA Healthcare System, Minneapolis, MN; Sioux Falls VA Healthcare System, Sioux Falls, SD; St. Cloud VA Healthcare System, St. Cloud, MN; Central Iowa VA Healthcare System, Des Moines, IA; Omaha VA Care Center, Omaha, NE; Black Hills VA Care Center, Hot Springs, SD, Black Hills VA Care Center, Fort Meade, SD. 4. The North American Industry Classification System (NAICS) code associated with this request for information is: 325120 which have a business size standard of 1,000 employees. 5. Responses to this RFI should be emailed to VHACOSAOCentralRegionContractStaff@va.gov not later than 3:30 pm CT, July 23, 2012. Please include "Regional Medical Gases" in the subject line of all correspondence. Responses should include the following information: a.Name and address of company b.Business size and socioeconomic status (i.e. SDVOSB, VOSB, 8a, etc.) c.Capabilities: a.Describe your ability to provide services to the entire geographic area as identified in paragraph 3, to include the following. MANUFACTURERS: i.Distribution network configuration (number of production facilities, distribution centers, warehouses (within the designated geographical area)). ii.Average number of Accounts Receivable greater than sixty (60) days from companies to which you are a supplier of medical gases, broken down by socioeconomic category of small or large business concerns for the last three years. NON-MANUFACTURERS: i.Distribution network configuration (number, location and network missions of suppliers (for end product), distribution centers, within the designated geographical area). ii.Number and name of established distribution partners within the designated geographical area. iii.Average number of Accounts Payable in excess of thirty (30) days for the last three years. iv.Number of primary staff currently used to manage medical gases sub-contracts. GENERAL: i.List the three largest contracts you company has been awarded within the last three (3) years, to include the aggregate dollar value and number of delivery points supplied. ii.Indicate if your company can provide services for the entire region specified or list the VISN's/Geographic area that can be serviced. iii.Any other pertinent information. 7.
