- Title: The School District of Philadelphia: Mold and Dampness Assessment NOTICE OF INTENT TO ISSUE A PURCHASE ORDER. The Centers for Disease Control and Prevention, National Institute for Occupational Safety and Health (NIOSH), Division of Respiratory Disease Studies (DRDS), Field Studies Branch, 1095 Willowdale Road, Morgantown, WV 26505, hereby announces its intent to issue a purchase order to The School District of Philadelphia for a computerized version of the NIOSH Dampness and Mold Assessment Form. It is NIOSH's goal to increase the awareness and capacity of schools throughout the country to assess dampness and mold by providing a simple observational tool, the "NIOSH Dampness and Mold Assessment Tool", in both hard copy and computerized form. Having already piloted the hard copy version of the form in a small sample of schools in the Philadelphia School District, we are now ready to pilot on a larger scale a computerized version of the "NIOSH Dampness and Mold Assessment Form". There are three requirements for this contract: 1) The School District of Philadelphia, in conjunction with the Philadelphia Federation of Teachers Health and Welfare Fund and Union, will use the new electronic, tablet based version of the "NIOSH Dampness and Mold Assessment Form" to conduct assessments in 70 schools within the PSD. This amounts to 14 days of testing for each of the 5 inspectors. NIOSH will provide onsite training to PSD staff and contractors in the use of the new computerized data collection system and will provide ongoing support as needed to complete the 70 school assessments. This will provide NIOSH a thorough assessment of the NDMAT data collection system. It is anticipated that this will be staged so that each inspector will complete a couple of schools, modifications will be made, a couple of more schools will be completed, and additional modifications will be made. This process will continue until all bugs are worked out of the software and all 70 schools have been completed. 2) NIOSH follows strict confidentiality procedures and data provided to NIOSH will only be used for the following purposes, unless further agreed upon by the PSD and the PFTHWFU. Data received by NIOSH will be blinded at the school level. Data will be used by NIOSH to: a) identify bugs in the electronic data collection software which includes the entry, edit, backup, data export, and reporting modules; b) verify that the electronic tablet-based data entry system is logging data correctly; c) develop a database for the PSD that will maintain all current and future data collected with the system; d) develop a reporting system that will provide simple reports upon completion of an inspection; e) develop an analysis system that will allow for simple analysis of the data collected. 3) The School District of Philadelphia will provide detailed feedback on the computerized data collection system so it can be further improved by NIOSH for broader Nation-wide distribution. Responsible sources that believe they possess the expertise and capabilities identified above are encouraged to submit to the Contracting Officer within 15 days from the posting date of this notice, their written capabilities statement and pricing information in the format they choose, not to exceed 10 pages. All vendors must be registered in the Central Contractor Registry (CCR) prior to an award of a federal contract. The website is: www.ccr.gov. The Government will review any/all capabilities statements and pricing information submitted and determine if other qualified sources do exist that could provide this requirement. Information received in response to this announcement will be used solely for the purpose of determining whether to conduct a competitive procurement. If no affirmative responses are received within 15 days, in accordance with FAR 13.106-1(b)(1), negotiations will be conducted with The School District of Philadelphia, 440 N. Broad Street, Philadelphia, PA 19130, as the only source and a purchase order will be issued without any additional notices being posted.
