Description
This is a combined solicitation/synopsis for commercial items prepared in accordance with the format in the Federal Acquisition Regulation (FAR) Subpart 12.6, as supplemented with additional information included in this notice. This announcement constitutes the only solicitation; quotes are being requested and a written solicitation will not be issued. This solicitation is issued as a Request for Quotation (RFQ), solicitation number FA4608-12-Q-S284, in accordance with FAR Parts 12 and 13, and is in accordance with provisions and clauses effective in Federal Acquisition Circular (FAC 2005-23 effective 26 Dec 2007). The North American Industry Classification System (NAICS) number is 813110, and the size standard is 7.0 million. The proposed acquisition is a total small business set-aside. The requirement is for a Protestant Religious Education Coordinator to perform requirements as stated in the Statement of Work (SOW). Offerors must comply with all instructions contained in FAR 52.212-1, Instructions to Offerors Commercial Items, applies to this acquisition and the following addenda applies. Offerors shall submit their quote on letterhead stationery, provide unit prices with a grand total, and the contractor must remit a completed copy of the REPRESENTATIONS AND CERTIFICATIONS located at FAR 52.212-3 unless registered in ORCA. FAR clause 52.212-4, Contract Terms and Conditions -- Commercial Items, applies to this acquisition. FAR clause 52.212-5, Contract Terms and Conditions Required To Implement Statutes Or Executive Orders -- Commercial Items, applies to this acquisition. Offers will be evaluated on the prices as offered. The Contracting Officer is not responsible for locating or obtaining any information not identified in the offer. If an offer cannot comply with every requirement, that offer will not be technically acceptable, and therefore will not be considered. Once an offer has been determined to be technically acceptable, and in compliance with all directions in this solicitation, the Government will award to the offeror with the lowest evaluated price. Quotes can be emailed to the attention of Amn Jesse Bozzella at jesse.bozzella@us.af.mil no later than 19 July 2012 at 4:00 pm Central time. Offers received after this date and time will be considered as late submissions in accordance with 52.212-1(f) and will not be evaluated or considered. When submitting bids please break down quotes by year and reaffirm the total price. The evaluation criteria is Lowest Price Technically Acceptable -Technical acceptability includes the following: -At least 2 years as a religious education coordinator or teacher - Must have a bachelors degree or at least 3 years of college with 2 years of experience in religious education programs -Ability to answer listed interview questions in an acceptable manner **The Interview Process will be held within 7 days after solicitation closing date at Barksdale AFB Chapel 1 between the hours of 8:00 a.m. and 4:00 p.m. Interviews will be either face to face or through teleconference. Face to face interview preferred but not mandatory. Other written material may be submitted to support oral presentation, but no media will be accepted. The evaluation criteria for the interview questioning process is either acceptable or unacceptable answers. In addition, the interview will be a maximum of one hour. IN COMPLIANCE WITH THE SERVICE CONTRACT ACT OF 1965, AS AMENDED, AND THE REGULATIONS OF THE SECRETARY OF LABOR (29 CFR PARA 4), THIS CLAUSE IDENTIFIES THE CLASSES OF SERVICE EMPLOYEES EXPECTED TO BE EMPLOYED UNDER THIS CONTRACT AND STATES THE WAGES AND FRINGE BENEFITS PAYABLE TO EACH IF THEY WERE EMPLOYED BY THE CONTRACTING AGENCY SUBJECT TO THE PROVISIONS OF 5 U.S.C. 534 OR 5332.
