Description
This is a combined synopsis/solicitation for commercial items prepared in accordance with the format in Subpart 12.6, as supplemented with additional information included in this notice. This announcement constitutes the only solicitation; proposals are being requested and a written solicitation will not be issued. (ii) Solicitation VA258-12-Q-0644 is issued as a request for quotation. (iii) This solicitation document and incorporated provisions and clauses are those in effect through Federal Acquisition Circular2005-36. (iv) This procurement is being issued as a 100% Small Business set-aside under NAICS code 561730 with a small business size standard of $7M. (v) CLIN 0001, (1) JB. (vi) The Contractor shall furnish all management, supervision, labor, transportation, materials and all other required items necessary to perform all operations in connection with the project titled 'Tree Trimming Services' at the Southern Arizona Veterans Affairs Health Care System facilities, Tucson, Arizona. Work will consist of trimming all palm trees, (approx. 139), trimmed at the 11-1 position; Trim five (5) Sumak trees located on the North side of building 60. These trees will be lifted to three feet off the roof tile and tree will be thinned out and balanced all around the canopy; Prior to leaving the site on a daily basis, all trimmings/debris will be picked up and removed from the facility; Contractor shall provide adequate traffic control in necessary areas; Tom Lorentz, SAVAHCS Gardner, is appointed the primary technical representative and the receiving official. All work must be approved and accepted by Mr. Lorentz, or alternate government official, as appointed by the Contracting Officer, prior to project completion; Contractor shall contact Mr. Lorentz at (520) 792-1450, ext. 1-6336, prior to beginning work to clarify the scope of work, discuss schedule/routing, and discuss any other appropriate matters. You will also contact Mr. Lorentz should you desire a pre-bid site visit; Period of Performance: Contractor shall have ten (10) calendar days to complete and work and ensure site is cleaned up from debris; Contractor shall submit a copy of their current insurance certificate with appropriate Arizona State liability rates. (vii) Period of Performance: 10 calendar days, with a completion not later than Aug 24, 2012. (viii) The provision at 52.212-1, Instructions to Offerors -- Commercial, applies to this acquisition. (ix) 52.212-2, Evaluation -- Commercial Items applies. Quotes will be evaluated on price. (x) Offerors must include a completed copy of the provision at 52.212-3, Offeror Representations and Certifications -- Commercial Items, with its offer. (xi) Clause 52.212-4, Contract Terms and Conditions -- Commercial Items, applies to this acquisition. (xii) Clause 52.212-5, Contract Terms and Conditions Required To Implement Statutes Or Executive Orders--Commercial Items to include clauses 52.209-5--Certification Regarding Debarment, Suspension, Proposed Debarment, and Other Responsibility Matters,52.209-6--Protecting the Governments Interest When Subcontracting with Contractors Debarred, Suspended, or Proposed for Debarment, 52.222-41-Service Contract Act, 52.232-33-Payment by Electronic Funds Transfer--Central Contractor Registration, 52.237-3--Continuity of Services, 52.247-34--F.o.b.- Destination, 52.222-19-Child Labor-Cooperation with Authorities and Remedies, 52.222-37-- Employment Reports on Special Disabled Veterans, Veterans of the Vietnam Era and Other Eligible Veterans, 52.222-42--Statement of Equivalent Rates for Federal Hires applies to this acquisition and 252.232-7003 'Electronic Submission of Payment Requests.'
