All proposals must be received by the contracting officer via e-mail at lisa.nooner@va.gov titled Fresenius 2008T Dialysis Unit. This is a combined synopsis/solicitation for commercial items prepared in accordance with the format in FAR Subpart 12.6, as supplemented with additional information included in this notice. This announcement constitutes the only solicitation; offers are being requested and a written solicitation will not be issued. The solicitation number, VA248-12-Q-1693 is issued as a Request for Quote (RFQ). This document and incorporated provisions and clauses are those in effect through Federal Acquisition Circular 2005-43 effective September 16, 2010 and Veterans Affairs Acquisition Regulation Supplement (VAAR) current to January 7, 2010. The North American Industry Classification System (NAICS) Code for this acquisition is 423450. Size Standard: 100 Employees. This procurement is not a set-aside. The proposals must be quoted ALL or none exact match only. *See Items List Below: 1.LI 001-2008T/19713-Fresenius 2008T Dialysis Unit with CDX (Clinical Data Exchange) System, Crit-Line III Monitor C(CLM) Order No. CL10021053, Crit-Line Pole Mount - Short Arm, Order No. CL90000023. 2.LI 002-For every ten (10) 2008T machines purchased, FUSA shall pay fifty percent (50%) of the airfare and one hundred percent (100%) of the hotel costs (room and tax only) for one (1) technical training class held in Coppell (Dallas), Texas. Training and materials fee shall be waived. Purchase shall be responsible for all other expenses associated with this technical training class (e.g., shuttle, meals).
