- (i) This is a combined synopsis/solicitation for commercial items in accordance with the format in FAR Subpart 12.6 as supplemented with additional information included in this notice. This announcement constitutes the only solicitation; proposals are being requested and written solicitation will not be issued. (ii) Request for Quote (RFQ) AG-0261-S-12-0063 is a combined synopsis/solicitation for commercial items. This combined synopsis/solicitation is issued as a RFQ. Submit written offers only, oral offers will not be accepted. Fax to 208-373-4168 or email to dearly@fs.fed.us. All firms or individuals responding must be registered with the Central Contractor Registration (CCR). (iii) This combined synopsis/solicitation document and incorporated provisions and clauses are those in effect through Federal Acquisition Circular 2005-59. It is the contractor's responsibility to be familiar with applicable clauses and provisions. Clauses and provisions can be found at: http://farsite.hill.af.mil/vffar1.htm (iv) This procurement is being issued as 100% Small Business Set-Aside. North American Industrial Classification Standard: 325320 Small Business Size Standards: 500 (v) Line Item (001) 4,000 lb of Basamid Granular Fumigant, Dazomet Brand or Equivalent (vi) Supply and deliver 4000 pounds Basamid granular fumigant, Dazomet brand name or equal (EPA Reg No. 5481-9027-70051), palletized, and in 50-pound bags (vii) FOB-Destination for delivery to: Deliver F.O.B. to Lucky Peak Nursery, 15169 E. Highway 21, Boise, ID 83716 (located 16 miles north of Boise on State Highway 21) on or before August 3, 2012. All deliveries shall be made on regular workdays (Monday-Friday) during regular business hours (7:30 am to 2:30 pm),excluding official Federal Government holidays. Contractor shall notify the Lucky Peak Nursery (208-343-1977) a minimum of 24 hours prior to delivery. Lucky Peak Nursery is located 16 miles north of Boise, ID on State Highway 21. Responsibility and Inspection: Unless otherwise specified in the contract or purchase order, the supplier is responsible for the performance of all inspection requirements and Quality Control (QC). (viii) (ix) The Government will award a firm-fixed price contract resulting from this combined synopsis/solicitation to the responsible offeror whose offer conforming to the synopsis/solicitation will be most advantageous to the Government, price and other factors considered. Award shall be based on All or None and Best Value to the Government. Best value can include but is not limited to price, delivery date, and technical capability. (ix) The clause at FAR 52.212-4 Contract Terms and Conditions-Commercial Items, applies to this acquisition. (x) The following provisions and clauses apply to this acquisition under FAR 52.212-4 Addendum: FAR 52.211-6 Brand Name or Equal; FAR 52.223-2; AGAR 452.204-71; and 452.209-71. (xi) The clause at FAR 52.212-5 Contract Terms and Conditions Required to Implement Statutes or Executive Orders, applies to this acquisition along with the following provisions and clauses under this clause: FAR 52.222-50; 52.233-3; 52.233-4; 52.209-10; 52.219-6; 52.219-28; 52.222-3; 52.222-19; 52.222-21; 52.222-26; 52.222-36; 52.223-18; 52.225-1; 52.225-13; 52.232-33; and 52.223-1 and AGAR 452.204-70. (xii) The provision at FAR 52.212-1 Instructions to Offerors-Commercial Items, applies to this acquisition. (xiii) The clause at FAR 52.212-3 Offeror Representations and Certifications - Commercial Items, applies to this acquisition. Offeror shall complete only paragraphs (b) of this provision; f the offeror has completed the annual representations and certificates electronically via https://orca.bpn.gov. If an offeror has not completed the annual representations and certifications electronically at the ORCA website, the offeror shall complete only paragraphs (c) through (o) of this provision. (xv) The clause at AGAR 452.209-70 and Alternate I Representation by Corporations Regarding an Unpaid Delinquent Tax Liability or a Felony Conviction (Deviation 2012-01). Offeror shall complete only paragraphs (b) of this provision and return with quote. (xvi) Quotes must be emailed to Diana Early, dearly@fs.fed.us), or faxed to (208) 373-4168. Quotes are required to be received no later than 16:30 MST July 16, 2012.
