This is a Sources Sought notice for information only. This is not a solicitation for proposals and no contract will be awarded from this announcement. In preparation for future procurement for the VISN 15 Network, the Network 15 Contracting Office is performing market research to gain knowledge of potential sources and their size classification (service disabled veteran owned small business, veteran owned small business, hub zone, 8(a), small disadvantage business, woman-owned small business, small business, or large business) relative to NACIS 334510 that can offer same or similar surgical or medical products listed below. Capable companies should submit an electronic statement outlining their companies capability and capacity to provide the product Federal Supply Schedule contract or Open Market the specification of the surgical or medical supplies and the delivery time to deliver the products to VISN 15 Medical Centers. Also firms should provide their business size status in accordance with the small business administration website (www.sba.gov) for the above referenced NACIS code that fall within. Please submit statements of capabilities electronically to Hans.Lorenzini@va.gov no later than Monday July 9 @ 0800 CST. Phone inquiries will not be accepted. Please reference VISN 15 1B Project Defibrillators and AEDs in the subject line of your e-mail. Vendor Checklist for this announcement, respond in following numbered format. 1. Vendor name: 2. FSS Contract Numbers: provide contract numbers and indicate your ability to offer the requested items 100% through a FSS contract on the attached sample statement of requirements. Indicate which Schedules and SINs your contract is listed under and the FSS contract price for the items on contract. 7. Capabilities Statement: provide model numbers, standard features and options of equipment that can be offered. If there are any specifications that your product cannot meet, indicate in separate section. 8. Comments: provide comments regarding the statement of requirements that may be pertinent to the improvement of the statement of requirements
